Esta pandemia le ha dado un giro a la enseñanza quirúrgica, ya que durante este periodo todos los esfuerzos de los servicios quirúrgicos se concentrarán en proporcionar la mejor atención posible a la población afectada por el COVID-19, al mismo tiempo que se protege y preserva la fuerza laboral quirúrgica. Sin embargo, la educación quirúrgica no debe ser abandonada y los programas de educación virtual quirúrgica continuada pueden ser una opción viable para mitigar el efecto de la pandemia en la formación de los médicos residentes. Al mostrar y compartir esta experiencia queremos abrir la puerta a desarrollar trabajos futuros para evaluar y comparar de forma objetiva la eficacia de estos nuevos métodos de enseñanza en nuestro país.
Artículo que expresa los aspectos más resaltantes de analizar la aplicación de pausas activas como estrategia preventiva de la fatiga y el mal desempeño laboral por condiciones disergonómicas en actividades administrativas de una empresa manufacturera ubicada en el municipio Valencia, estado Carabobo, con el propósito de prevenir enfermedades ocupacionales, identificando condiciones disergonómicas y generar bienestar en los trabajadores en pro del desempeño laboral. Metodológicamente se trató de una investigación de campo descriptivo, se utilizó como técnicas de recolección de datos, la observación directa y laencuesta, en conjunto con los instrumentos: guía de observación y cuestionario los cuales fueron aplicados a una muestra de (21) trabajadores del área administrativa. Como resultado se recogió una alternativa para aumentar el desempeño laboral, disminuir dolencias físicas, estrés, desórdenes emocionales y riesgos laborales asociados a la actividad diaria.
ResumenIntroducción. Se han reportado hallazgos incidentales durante la cirugía bariátrica, como tumores gástricos (0,6 a 0,8 %), entre los cuales el más frecuente es el del estroma gastrointestinal (GIST). Es importante tener en cuenta que el pilar del tratamiento de este tipo de tumores es la cirugía con márgenes negativos. Métodos y resultados. Se llevó a cabo un estudio retrospectivo, descriptivo y observacional, desde enero de 2013 hasta junio de 2016; durante este periodo, 320 pacientes fueron sometidos a cirugía bariátrica, manga gástrica en 250 y derivación gástrica en 70, en la Clínica Universitaria El Bosque. En tres de ellos, dos con manga gástrica y uno con derivación gástrica, se encontró incidentalmente un tumor gástrico del estroma gastrointestinal (GIST) con confirmación por histopatología; mediante escisión, se obtuvieron bordes negativos para tumor, no se presentaron complicaciones ni se prolongó la estancia hospitalaria. Discusión y conclusiones. Teniendo en cuenta la ubicación de la lesión, es posible la intervención laparoscópica de los tumores gástricos del estroma gastrointestinal de 2 cm o más, encontrados durante un procedimiento de cirugía bariátrica. AbstractIntroduction: Incidental findings are reported in bariatric surgery, such as gastric tumors in 2% to 8%, gastrointestinal stromal tumors (GIST) being the most frequent. The pillar of handling this type of tumor is surgery with negative margins. Methods and Results: This is a descriptive retrospective study covering the period January 2013 to June 2016 during which 320 patients underwent bariatric surgery, 250 gastric sleeve and 70 gastric bypass, among which three had the incidental finding of gastric GIST confirmed by pathology; two patients had gastric sleeve and one gastric bypass; resection of the tumor was carried out obtaining adequate negative margins with no morbidities and no prolonged hospital stay. Discussion and Conclusions: Gastric sleeve and gastric bypass are the preferred surgical options in bariatric surgery, allowing resection of the incidentally found gastrointestinal stromal tumors for curative treatment, without altering the outcome or purpose of the bariatric surgery procedure.
Introducción. La hernia de Amyand es una hernia inguinal que contiene el apéndice cecal normal o en un estado de apendicitis aguda. Fue descrita por primera vez en 1735 por Claudius Amyand. La apendicitis aguda probablemente se debe a una obstrucción extraluminal generada por aumento de la presión en el cuello herniario que limita el flujo sanguíneo, en lugar del aumento de la presión intraluminal que se presenta en la apendicitis no asociada con hernias inguinales. Esto resulta en inflamación y proliferación bacteriana, y el apéndice cecal se torna más vulnerable al trauma. Métodos y resultados. Se presenta y analiza, clínica y quirúrgicamente, un caso de hernia de Amyand manejado en el servicio de cirugía general de una institución de tercer nivel de Bogotá; además, se hace una revisión de la literatura científica. El abordaje quirúrgico difirió del propuesto en la guía de manejo de Losanoff y Basson y, sin embargo, se obtuvieron resultados exitosos. Discusión. La hernia de Amyand es una entidad quirúrgica rara, la cual representa un reto diagnóstico y terapéutico, ya que se presenta con características que varían según dos procesos patológicos simultáneos: la hernia inguinal y la apendicitis aguda, cuando esta está presente. El abordaje quirúrgico depende de los hallazgos intraoperatorios, como perforación del apéndice cecal, fístula o pelviperitonitis. Por lo tanto, decidir cuál abordaje quirúrgico es el más apropiado depende de los hallazgos y las preferencias del cirujano. Conclusiones. El tratamiento con apendicectomía, reparo del defecto herniario con malla o ambos, ha suscitado discusión entre los cirujanos por muchos años. Las diferencias de opiniones sobre la técnica quirúrgica más apropiada se deben, entre otras cosas, a la incidencia baja de la hernia de Amyand y a las preferencias quirúrgicas de cada cirujano; el mejor abordaje quirúrgico todavía está en discusión. Sin embargo, la apendicectomía está contraindicada en ciertos pacientes cuando no existe inflamación y, cuando está presente, el uso de la malla todavía es controversial. Por lo tanto, cada reporte de hernia de Amyand y su tratamiento es importante, pues ofrece conocimiento sobre la técnica quirúrgica más apropiada para esta entidad.
