A remoção cirúrgica de terceiros molares inferiores impactados cujas raízes dentárias estão próximas ao canal do nervo alveolar inferior (NAI) representa um procedimento de alto risco para o desenvolvimento de lesão nesta estrutura anatômica. A lesão do NAI é uma complicação rara, muitas vezes grave e que pode ocorrer como consequência da extração de terceiros molares inferiores. A sua incidência é viável 0,5% a 8%, mas pode aumentar para 20 a 50% quando as raízes fazem contato com o osso cortical do NAI. A técnica da coronectomia foi proposta como uma alternativa a remoção completa de um terceiro molar inferior impactado localizado próximo ao NAI. A técnica consiste em remover apenas a coroa do dente, deixando a raiz no alvéolo, teoricamente evitará danos diretos ou indiretos ao NAI. Este trabalho propõe realizar uma revisão narrativa da literatura buscando discutir sobre a eficácia da coronectomia em relação à transtornos sensoriais, quando da remoção de terceiros molares inferiores impactados e desfavoravelmente posicionados em relação ao nervo alveolar inferior. As evidências sobre os resultados de longo prazo da coronectomia, permitem afirmar que se trata de um método seguro e eficaz nos casos em que o terceiro molar inferior se encontra em posição de alto risco de complicação envolvendo o NAI.
