LAMARTINE DE ASSIS (2) e V. ARRUDA ZAMITH (3)O presente caso foge ao habitual em virtude de embaraços de ordem diagnóstica e tático-cirúrgica. Estas dificuldades surgiram não somente em relação ao diagnóstico etiológico, mas também ao topográfico por ter havido discordância entre a sintomatologia clínica e a peri-miélografia. çado, apesar das dificuldades apontadas e da gravidade do comprometimento medular. Relata estar doente ha cerca de 4 anos. A moléstia iniciou-se bruscamente com súbita e passageira dor na região escapular esquerda. Alguns dias depois, a dor se repetiu na mesma região e com os mesmos característicos. Desde então, a dor surgia de vez em quando, sempre na mesma região e com os mesmos característicos, isto é, contínua, não muito forte, sem qualquer propagação e de curta duração (48 horas). Tal sintoma se repetiu cada 2 ou 3 mezes. Um ano mais tarde, notou diminuição da atividade, do apetite e da potência sexual. Assim passaram-se 24 mezes. Em janeiro deste ano, portanto ha 7 mezes, começou a sentir fraqueza nos membros inferiores, irradiação da dor escapular, agora intensificada e persistente, para a face anterior do tórax. Essas dores se agravavam pela tosse e espirro. Ao mesmo tempo, novos fenômenos álgicos surgiam na coluna dorsal, com irradiação para a face posterior do pescoço. Ha 3 mezes a moléstia adquiriu um aspecto deveras sombrio: fraqueza tal nos membros inferiores, que a deambulação tornou-se impossivel; parestesia no braço esquerdo, com nítida diminuição da força muscular nos dedos anular e médio da mão esquerda; parestesia nos pés e pernas; automatismo nos membros inferiores; retenção de fezes e urina. Nessa situação procurou este hospital onde teve infecção intestinal com febre alta (40°) durante 3 dias, depois do que atravessou um curto período de melhoras chegando até a andar. Dos antecedentes familiares e hereditarios ha a notar ausência de moléstias neur como infecções gerais. Dos antecedentes pessoais, é preciso salientar que, cm 1930 teve fraqueza nos membros inferiores (sic) e, em 1927, blenorragia e otite. A individualização dos glioblastomas multiformes entre as numerósas variedades de neoplasias cerebrais é obra já indiscutivelmente realizada como resalta da leitura dos tratados mais recentes que estudam a questão. É bastante vasta a bibliografia extrangeira sobre o assunto e, entre nós, alguns trabalhos de valor têm surgido, chamando a atenção, especialmente dos clínicos, sobre a necessidade do conhecimento da conduta bastante particular desse tipo de tumores. Assim, citamos o trabalho de Aluizio Marques e Rita Cardoso (1) em. que, a proposito de uma observação anátomo-clínica, fazem uma síntese das caracteristicas principais dos glioblastomas multiformes. De maior vulto, porem, é o trabalho de Jarbas Pernambucano (2), elaborado no Departamento de Anatomia Patológica da Faculdade de Medicina da Universidade de São Paulo, monografia em que são estudados 16 casos anátomo-clínicos com importantes conclusões que interessam ao clínico, tanto quanto ao patologista, e na qual abundante bibliografia é apresentada. Nesse trabalho figuram alguns casos cedidos por nós, do Arquivo do Serviço de Neurologia da Faculdade, em numero de 5 (casos 1, 2, 13, 15 e 16), alem de outro (caso 11) da clínica particular de um de nós.No presente trabalho contentamo-nos em menciona-los apenas, podendo sua exposição ser apreciada naquela tese. Fazemos todavia excepção para um deles que relataremos mais adiante sob n.° 2, pois este, tendo sido operado, completa o numero de 5 que tantas são as nossas observações neurocirúrgicas.
