The use of sedoanalgesia protocols in procedures in the pediatric emergency room should guide the professional in the choice of medication, the appropriate material, and in the evaluation of discharge criteria, thus assuring quality in care.
Our objective was to evaluate intestinal ischemia-reperfusion injury in growing rats, modulated by hypothermia (I/RH) and N-acetylcysteine (NAC). We used 30 EPM-1 Wistar male rats, aged around 35 days, weighing 90 g. Rats were randomized into 5 groups with 6 animals in each: I/RH group, intestinal ischemia under hypothermia for 40 min and reperfusion for 30 min; I/RH-NAC group, same procedure but adding NAC (150 mg x kg(-1)), previously with ischemia; S-H group, topic hypothermia for 40 min, and observation for 30 min; I/R H-Ve group; and S-NAC group, NAC administration and observation for 70 min. All animals were heparinized and anesthetized with ketamine (60 mg kg(-1)) and xylazine (10 mg kg(-1)) intramuscularly. Surgical procedures were done under microsurgical technique (augmentation, 10x). After laparotomy, the superior mesenteric artery was dissected and clamped to promote ischemia. Topic hypothermia was obtained by using plastic bags at 4 degrees C, changed every 10 min. Rats were sacrificed by exsanguination, and blood samples were utilized to measure D(-)lactate. Disponível em URL: http://www.scielo.br/acb. RESUMO Objetivo: Avaliar a função hepática de ratos hipertensos submetidos a baixa ou alta pressão de pneumoperitônio. Métodos: Foram utilizados 60 ratos, distribuídos em dois grupos (n= 30), de acordo com a pressão de pneumoperitônio de 4 e 10mmHg, sendo 15 animais da linhagem EPM-1 Wistar e 15 animais da linhagem SHR. Logo após a pesagem, os animais foram submetidos à anestesia com cetamina e xilazina intramuscular e, em seguida, submetidos a pneumoperitônio, durante 1 hora e desinsuflação por mais 1 hora. Foram coletadas amostras sangüíneas logo após a realização do cateterismo da artéria femoral esquerda, após uma hora de insuflação, assim como, depois de uma hora de desinsuflação. Estas amostras foram encaminhadas para a avaliação das enzimas hepáticas (AST, ALT, DHL) e bilirrubinas (total, direta e indireta). O nível de significância estatístico foi estabelecido em 5%. Resultados: Com pneumoperitônio de 4mmHg a AST e ALT apresentaram níveis iniciais menores do que após insuflação e desinsuflação enquanto a DHL, a BT e a BD não se alteraram; em ambas as linhagens. Na pressão de 10mmHg a AST e a DHL não se alteraram nos ratos normotensos e aumentaram nos hipertensos, a BD não se alterou, a ALT e a BT aumentaram para ambas as linhagens. Conclusão: A função hepática mostrou-se alterada e a medida do tempo após a desinsuflação foi curta para evidenciar uma eventual reversão dessas alterações, tanto nos animais hipertensos, como em animais hígidos. Na síndrome de isquemia e reperfusão os pulmões podem ser alvo de lesão a distância como nos casos de choque, trauma ou ainda nos casos de transplante hepático. Objetivo: Avaliar o efeito protetor da N-acetilcisteína (NAC) sobre os pulmões após isquemia hepática. Métodos: Foram utilizados 12 ratos, machos, linhagem EPM-1 Wistar, separados aleatoriamente em dois grupos com seis animais (controle e experimento). Os animais de ambos os grupos foram submetidos à anestesia com cloridrato de quetamina e cloridrato de xilazina. Realizou-se a incisão mediana longitudinal, identificação do hilo hepático e da veia cava caudal. Quinze minutos antes do clampeamento injetou-se solução glicosada a 5% no grupo controle e NAC diluída em solução glicosada a 5% no grupo experimento. Os animais foram mantidos em isquemia hepática durante 30 minutos, sendo em seguida realizada toracotomia e remoção cirúrgica dos pulmões para avaliação histológica com coloração pela hematoxilina-eosina. Resultados: A análise dos cortes do parênquima pulmonar mostrou semelhança nos dois grupos estudados, ocorrendo colapso alveolar, infiltrado neutrofílico, congestão vascular e áreas hemorrágicas, compatíveis com a repercussão sistêmica da isquemia hepática. Conclusão: A NAC não modifica a lesão pulmonar decorrente da isquemia, à microscopia óptica.
In ischemia and reperfusion syndrome, the lungs can be the target of remote lesion as it happens in cases of shock, trauma, or liver transplantation. Objective: The aim was to evaluate the effect of N-acetylcysteine (NAC) in the lungs following liver ischemia. Methods: 12 male EPM-1 Wistar rats were used, divided into two groups of six animals each (control and experimental). Animals of both groups received anesthesia with ketamine and xylazine. Longitudinal medial incision was performed for identification of the hepatic hilum and caudal vena cava. 5% glucose solution was injected in the control group 15 minutes before hepatic hilum clamping, and NAC dissolved in the same solution was injected in the experimental group. The animals were maintained in hepatic ischemia during 30 minutes, followed by surgical resection of the lungs for histological evaluation, stained by HE. Results: Cavity collapse, neutrophil-permeation, vessel congestion, and hemorrhagic areas were observed which are compatible with systemic repercussion of liver ischemia, in all animals of both groups. Conclusion: NAC does not protect lungs from morphological changes following liver ischemia
