Introducción: el trauma cerrado de abdomen en pediatría ocasiona lesión de órganos abdominales en 8-12 % de la población, siendo lesiones duodenales solo el 10 % de estas, por lo que son poco frecuentes en pediatría. La principal causa de lesión duodenal se relaciona con abuso infantil, siendo los accidentes de vehículos de motor el segundo mecanismo más común. Cuando se trata de lesiones no destructivas, la reparación primaria del duodeno es razonable. Sin embargo, las lesiones destructivas ameritan estrategias de reparación más complejas. Material y Método: presentamos paciente masculino de tres años edad con lesión duodenal grado III en ángulo duodenoyeyunal, la misma fue intervenida inicialmente mediante anastomosis primaria, presentado posterior dehiscencia. En su segunda intervención se realiza cierre duodenal distal y anastomosis gastroyeyunal, favoreciendo flujo retrogrado de vías biliares a estómago, con adecuada evolución. Conclusión: existen múltiples abordajes quirúrgicos, los cuales representan aun hoy gran morbilidad. El cierre duodenal con la posterior decisión de cómo derivar las vías biliares, es probablemente el mayor desafío. Se plantea la posibilidad del cierre duodenal distal a las vías biliares, con drenaje retrógrado. Por lo cual la gastroyeyunostomía es una técnica alternativa en pediatría en la reparación de lesiones complejas del duodeno.
La peritonitis meconial secundaria a perforación intestinal es una complicación temida de la atresia intestinal, puesto que incrementa la morbimortalidad perinatal. Aunque sigue siendo una presentación rara, su diagnóstico debe de hacerse temprano para evitar complicaciones. Presentamos un caso clínico de un recién nacido de 38 semanas sin antecedentes prenatales indicativos de obstrucción intestinal. Quien presenta vómitos biliosos/ fecaloideos, no meconización a las 12 horas de vida extrauterina, por lo cual se le realiza radiografía de abdomen de pie y acostada que muestra marcada distensión de asas intestinales, ausencia de aire distal, edema interasas. Se realiza laparotomía exploratoria y se hace yeyunostomía para luego re-intervenirla (second look), encontrando mejoría de asas intestinales, logrando una anastomosis yeyuno-ileal término-terminal. La operación temprana se puede considerar un factor de importancia para mejorar el resultado de los pacientes con peritonitis por meconio, de igual forma se necesita de un diagnóstico temprano.
Introducción: la lesión esofágica por cáusticos debida a la ingestión de álcalis o, con menos frecuencia, ácidos, puede ser accidental en niños o por intento de suicidio en adolescentes y adultos. La secuela esofágica de la ingestión de un cáustico es una cicatriz en la pared del esófago que se relaciona con frecuencia con la formación de estenosis intensa y en muchas ocasiones, se requiere de un conducto que restablezca la función del esófago. Material y método: en el siguiente trabajo se expone el caso clínico de un paciente masculino de 7 años de edad, presentando sialorrea y dificultad para la deglución debido a ingesta de sustancia cáustica de manera accidental. Se realizó endoscopia que reporta estenosis esofágica de 2 mm, para garantizar su alimentación se le practicó una gastrostomía. No fue posible el esquema de dilatación, con la necesidad de proceder a una sustitución esofágica por tubo gástrico invertido vía retroesternal. Presentó una complicación postquirúrgica de neumotórax. Una estancia de 12 días con inicio de la vía oral a los 11 días de postquirúrgico, previa realización de esofagograma. Conclusión el propósito consiste en describir la presentación y el progreso clínico del paciente luego de esta técnica de sustitución esofágica por tubo gástrico invertido vía retroesternal.
Introducción: la duplicación gastrointestinal es una ano-malía relativamente rara que puede ocurrir en cualquier nivel desde la cavidad bucal hasta el recto, los casos de duplicación gástrica son aún más raros, y la mayoría de ellos se han reportado en niños. Material y método: en el siguiente trabajo se expone dos casos; número 1: paciente de sexo femenino de 2 años de edad, con presentación clínica de vómitos de contenido alimentario que luego se tornaron biliosos en número de 3-4 ocasiones por día, de 2 días de evolución, dolor abdo-minal difuso de 2 días de evolución, masa abdominal localizada en hemiabdomen derecho. Caso número 2: paciente masculino de 4 días de vida, a quien se realiza ultrasonido obstétrico a las 34 semanas de gestación, donde se evidencia imagen quística nivel abdominal. A ambos casos se le realiza laparotomía exploratoria eviden-ciando en caso número 1: duplicación gástrica al que se le realiza exéresis total y rafia gástrica, y el caso número 2: malrotación intestinal con vólvulo de intestino medio más duplicación intestinal (ileal), al que se le aplica proce-dimiento de Ladd más resección y anastomosis termino terminal ileoileal, ambos pacientes evolucionan de manera satisfactoria. Conclusión: el propósito consiste en describir la presen-tación de ambos casos en la infancia, lo importante de los métodos diagnósticos y de la corrección quirúrgica por diferentes técnicas según la localización del quiste en el tracto gastrointestinal.
