El síndrome de Parkes Weber es una patología congénita e infrecuente, cuya incidencia es de 1:100,000 personas; se manifiesta por la tétrada que incluye malformaciones vasculares, várices, crecimiento excesivo de partes blandas-tejido óseo y manchas en vino de oporto. El diagnóstico es clínico y se confirma mediante estudios imagenológicos. Presentamos el caso de un neonato que al examen físico presenta manchas en vino de oporto en extremidades inferiores y genitales, además de hipertrofia de tejidos blandos. El examen doppler de extremidades inferiores que reporta malformación arteriovenosa en ambas extremidades y flujo estásico. La angiotomografía de miembros inferiores reporta hipertrofia de tejidos blandos y malformación arteriovenosa en vasos poplíteos y vasos linfáticos. Paciente falleció a sus 14 días de vida por coagulopatía y choque séptico. El manejo es conservador, el pronóstico de vida suele ser desalentador, debido a las complicaciones que suelen ser frecuentes.
