RESUMENIntroducción: La abdominoplastia es un procedimiento quirúrgico complementario al abordaje de las hernias incisionales complejas. Pocos estudios comparan las complicaciones tempranas en la práctica clínica.Objetivo: Conocer la morbilidad asociada a la realización de abdominoplastia y a la corrección de la hernia incisional compleja en la misma operación.Material y métodos: Estudio de cohorte en pacientes con reparación de hernia incisional electiva con técnica de separación posterior de componentes y seguimiento a 30 días. Se comparan pacientes con abdominoplastia con los que no la tuvieron. Las variables analizadas se clasificaron según las características sociodemográficas, clínicas y quirúrgicas de cada grupo.Resultados: Se incluyeron 203 registros clínicos. Se realizó abdominoplastia a 45 pacientes en la misma operación de la reparación de la hernia. La media de edad para ambos grupos fue de 57 años y el 51.7 % eran mujeres. La media de índice de masa corporal fue de 30.57 kg/m 2 para el grupo de abdominoplastia y de 27.84 kg/m 2 en el grupo comparación. El 28.9 % del grupo de abdominoplastia y el 31 % de los pacientes sin abdominoplastia presentaron complicaciones tempranas.La infección del sitio operatorio fue el evento más frecuente. No se identificó diferencia estadísticamente significativa en las complicaciones (RR crudo: 0.90 [IC 95 %,], RR ajustado: 1.09 [IC 95 %, 0.60-1.98]).
Conclusiones:No hay diferencias en las complicaciones a 30 días cuando se realiza la abdominoplastia y la reparación de hernia incisional en la misma operación. La realización simultánea de estos procedimientos en pacientes específicos podría evitar otra intervención adicional.
Factores asociados a recidivas en pacientes sometidos a cirugía de separación de componentes posterior Associated factors to recurrences in patients undergoing posterior component separation surgery
La Hernia de Spiegel es un tipo de hernia de la pared abdominal poco frecuente. Una vez se diagnostica es conveniente intervenir por el alto riesgo de encarcelación y complicación. A continuación presentamos el caso clínico de una paciente con antecedente de fibrilación auricular anticoagulada con Apixaban; Consultó al servicio de urgencias con signos y síntomas de obstrucción intestinal y se confirmó por tomografía de abdomen la presencia de Hernia de Spiegel izquierda encarcelada. Decidimos esperar 24 horas luego del consumo de anticoagulante para intervenir a la paciente por el alto riesgo de sangrado, la espera podía producir en la paciente una perforación intestinal; por lo que no consideramos oportuno esperar 48 horas sin el consumo de anticoagulante a pesar de ser la recomendación. El abordaje se realizó vía laparoscópica con técnica IPOM, buscábamos evitar sangrados masivos ya que vía laparoscopia se produce un menor trauma en los tejidos. La paciente no presentó complicaciones, tampoco presentó sangrados masivos en la cirugía ni en el postoperatorio. Han transcurrido 13 meses luego de haber sido intervenida y hasta el momento sin reaparición de su hernia.
Introducción. El objetivo de este estudio fue describir las características de la población y determinar las principales complicaciones de los pacientes que son llevados a cirugía por hernia lateral en el abdomen.
Métodos. Estudio observacional retrospectivo, que incluyó a los pacientes sometidos a herniorrafia lateral, entre 2015 y 2019 en un centro de tercer nivel. La información se obtuvo del registro de historias clínicas. Las variables analizadas se clasificaron según las características sociodemográficas de los pacientes, clínicas y quirúrgicas. Se hizo una caracterización de acuerdo con los resultados encontrados.
Resultados. Se incluyeron 51 pacientes con hernia lateral, 29 de ellos mujeres, con un promedio de edad de 59 años y de índice de masa corporal de 28 kg/m2 . El 60 % tenía comorbilidades de base, siendo la hipertensión arterial la más frecuente. La mayoría se clasificaron como ASA II. En cuanto a la localización, la L3 fue la más común (37,2 %). Se presentaron complicaciones postquirúrgicas en el 27,4 % de los pacientes, siendo las más frecuentes seromas, hematomas e infecciones de sitio operatorio. Ningún paciente requirió reintervención para el manejo de las complicaciones.
Conclusión. Conocer la población y determinar cuáles son las principales complicaciones postquirúrgicas de un procedimiento permite tomar medidas para disminuir su frecuencia, pero en este caso, se necesitan estudios adicionales para determinar cuáles son los mayores factores asociados a las complicaciones.
Introducción: La evisceración posquirúrgica es una complicación con origen multifactorial.Los estudios para prevenirla modifican la técnica de cierre. La técnica reinforced tensión line (RTL) se ha usado en el tratamiento de hernias incisionales.Material y métodos: Ensayo clínico en pacientes sometidos a laparotomía media. Se formaron dos grupos: 1) control con cierre habitual, y 2) grupo experimental con la técnica RTL. Se evaluó la presencia de evisceración posquirúrgica.Resultados: Terminaron el estudio 89 pacientes. Se presentaron un total de 9 (20 %) y 2 (4.5 %) evisceraciones, con diferencia a favor de la técnica RTL con una p de 0.0273.Conclusiones: Se encontró que la técnica RTL reduce la incidencia de evisceración posquirúrgica.
