Comparamos un grupo de 490 primigestantes primíparas adolescentes, de 18 años de edad o menos, con 471 primigestantes primíparas de 19-30 años.Encontramos dos tipos de diferencias: unas clínica y estadísticamente significativas. Otras sólo clínicamente importantes. En cuanto a las primeras, está la mayor incidencia de unión inestable y de pacientes sin control prenatal entre las adolescentes. La morbilidad gestacional fue mayor entre las adolescentes en unión inestable (p<0.05).Hubo también mayor incidencia de amenaza de parto pretérmino (p = 0.023) y de sufrimiento fetal (p =0.045) entre las adolescentes que en el subgrupo testigo de 19-24 años.En cuanto a diferencias importantes con significancia clínica más no estadística encontramos: mayor incidencia entre las adolescentes, de atonía uterina, sufrimiento fetal intraparto, y parto disfuncional, que en las de 19-24 años y de preeclampsia-eclampsia, amenaza de parto pretérmino, recién nacido pretérmino, bajo peso para la edad gestacional y RCIU que en el grupo de comparación de 25-30 años.La incidencia de preeclampsia-eclampsia en adolescentes primigestantes primíparas fue en nuestro estudio de 9.2%, 2.2 veces mayor que el promedio de 4.1% reportado por Pardo-Uriza (2) en su estudio cooperativo de 11 Instituciones colombianas.
Se hace el estudio retrospectivo de 91 embarazos molares, 6 casos de corioma destruens y 11 coriocarcinomas, que se presentaron en el Departamento de Gineco-Obstetricia del Hospital Universitario de Caldas en el lapso del 1º de Julio de 1963 al 30 de Junio de 1971.Se presentan los resultados del estudio y algunos comentarios sobre la literatura de los últimos 3 años.Se formulan algunas conclusiones y recomendaciones.
Se propone un modelo de registro precodificado para parto y puerperio, que permite procesar la información en computador.Se destacan sus características, que constituyen respuesta o solución a muchas de las más graves limitaciones y defectos de los registros tradicionales.Se anotan sus fallas y limitaciones.Se hace un comentario breve sobre los resultados del estudio, comparándolos con los obtenidos en 1977 en una muestra pequeña del mismo hospital, y los del estudio cooperativo correspondiente realizado en el mismo año por la CCRP.Se subrayan los siguientes resultados: porcentaje de pacientes de áreas rurales, aumento del porcentaje de analfabetismo, porcentaje importante (30.8%) de pacientes de riesgo mayor por su edad, aumento en el uso de anestesia general y conductiva, incremento notable del porcentaje de cesáreas, empleo escaso del bloqueo pudendo, incongruencia entre teoría y práctica en los tipos de episiotomía empleados.
Se revisaron las historias clínicas de 281 pacientes sometidas a biopsia aspirativa con aguja fina (BACAF) de la mama, en el período comprendido entre el 1 de julio de 1989 y el 30 de noviembre de 1995. Se seleccionaron 91 casos en los que hubo estudio histológico posterior. La correlación del diagnóstico por BACAF con el diagnóstico histopatológico final condujo a los siguientes resultados: sensibilidad 88.9%, Especificidad 75.7%, Valor predictivo de la prueba positiva (VPP) 84.2%, Valor predictivo de la prueba negativa (VPPN) 82.3%
Se estudian 460 pacientes esterilizadas por laparoscopia-electrocoagulación y 69 por minilaparotomía-Pomeroy.Se describen las características más sobresalientes de las usuarias y las complicaciones inmediatas de los procedimientos.Se comparan nuestros resultados, con algunos de los encontrados en la literatura.Se destaca la importancia de progresos técnicos, como el de los sistemas bipolares con corriente de alta frecuencia y el de la termocoagulación (endocoagulación).Se insiste en las ventajas que la minilaparotomía tiene sobre la electrocoagulación laparoscópica en el medio colombiano y la conveniencia de fomentar su uso.
