Introdução: a anemia de Fanconi (AF) é uma doença rara e heterogênea, considerada a causa herdada mais comum de falência da medula óssea. Objetivos: descrever um paciente diagnosticado com AF, cujo motivo do encaminhamento foi suspeita de neurofibromatose do tipo 1 (NF1). Material e métodos: realizou-se o relato do caso, a partir das informações obtidas do prontuário do paciente, junto com uma revisão da literatura. Nesta, foram utilizados os descritores Mesh e a base de dados PubMed/MEDLINE, sendo incluídos estudos em português/inglês publicados após 2007. O tempo de desenvolvimento do trabalho foi de cerca de 1 mês. Resultados: o paciente era um menino de 4 anos encaminhado por suspeita de NF1. Ele era filho de pais jovens e não consanguíneos. A mãe apresentava também manchas café com leite. O paciente nasceu de parto cesáreo, à termo, pesando 2450 gramas, medindo 44,5 cm, com perímetro cefálico de 33 e escore de Apgar de 8 no quinto minuto. Logo a seguir, realizou-se o diagnóstico de atresia de esôfago. A radiografia de tórax revelou cardiomegalia e anormalidade dos arcos costais superiores direitos. A nasofibrobroncoscopia verificou traqueomalácia do terço médio da traqueia. Ao seu exame físico, aos 4 anos, observou-se baixa estatura; dismorfias faciais que incluíam epicanto bilateral, raiz nasal alargada e estenose do conduto auditivo; clinodactilia bilateral de quintos dedos; polegares baixo implantados e manchas café com leite. A ecografia abdominal mostrou que o rim esquerdo era pélvico. A criança evoluiu com múltiplas internações por febre, cansaço e equimoses, sendo que se observou a presença de hepatoesplenomegalia e de linfadenomegalias. A avaliação hematológica evidenciou um quadro de pancitopenia. O estudo de quebras cromossômicas induzidas pelo diepoxibutano (DEB) revelou um aumento delas, além da presença de rearranjos entre os cromossomos, o que foi compatível com o diagnóstico de AF. Conclusão: a AF caracteriza-se por uma grande variabilidade fenotípica, o que pode levar a um atraso no seu diagnóstico. É interessante notar que as manchas café com leite, que foram o motivo do encaminhamento do paciente, são comuns na NF1 e consistem em um achado dermatológico descrito em pacientes com anemia de Fanconi.
