İncil’de İsa’ya dair anlatılan hikâyeler Batı Sanatı’nda bir hayli önem arz etmektedir. Sanatçıların birçoğu bu hikâyeleri tuvallerine yansıtmışlardır. İsa’nın havarileriyle birlikte yediği son akşam yemeği tarih boyunca birçok ressam tarafından malzeme olarak kullanılmıştır. Giotto di Bondone, Bouts, Poussin ve Leonardo Da Vinci bu ressamlardan bazılarıdır. Romalı askerlerce tutuklanmasından bir gün önceki bu akşam yemeğinde İsa havarilerinden biri tarafından ihanete uğrayacağını açıklamıştır. Bu noktada bir hayli dramatik tarihsel bir olay niteliği taşımaktadır. Dali ise bu anlatıyı dramatik tarihsel bir yemek şemasından ziyade liturjik bir yemek şeması olarak yorumlamış, son akşam yemeğinin huzurla devam eden komünyon aşamasını vurgulamıştır. Bu çalışmada Erwin Panofsky’nin “İkonografik ve İkonolojik Sanat Eleştirisi” yöntemi göz önüne alınarak 20. yüzyılın en etkili ressamlarından Salvador Dali’nin “Son Akşam Yemeği Ayini” adlı eseri analiz edilmiştir. Eser analiz edilirken doğal anlam, uzlaşmalı anlam ve içsel anlam alt başlıkları kullanılmıştır. Yapılan literatür taramasında daha önce Salvador Dali’nin özellikle Katolik inancını yoğun bir şekilde yaşadığı dönemde yaptığı bu dini konulu resimle ilgili bir takım yorumlamaların yapıldığı görülmüş, ancak Panofsky’nin ikonografik ve ikonolojik sanat eleştirisi yöntemiyle kapsamlı ve bütünlüklü bir analizinin yapılmadığı fark edilmiştir. Bu hususta işbu akademik çalışmanın literatüre katkıda bulunacağı düşünülmektedir.
ÖZETAmaç: Akut Romatizmal Ateşi (ARA) olan hastalarda primer ve sekonder penisilin profi laksisinin etkinliği değerlendiril-miştir. Gereç ve Yöntemler:Pediyatrik kardiyoloji polikliniğinde Kasım 1997-Ağustos 2012 tarihleri arasında ARA tanısıyla izlenen hastalar retrospektif olarak demografi k ve klinik özellikleri açısından incelenmiştir.Bulgular: Çalışmaya ARA tanısı alan 71 hasta dahil edildi. Hastaların %53.5'i kız, %46.5'i erkekti. Hastaların ortalama yaşları 10.8±3.0 yıl'dı. En sık etkilenim kardiyak olup hastaların %90`nında görülmüştür. Karditli olguların %96`sında mitral kapak tutulumu gözlenmiştir. En sık tutulum şekli mitral yetersizliği (MY) ve aort yetersizliğinin (AY) birlikteliği olup, 33 (%51.6) hastada gözlenmiştir. ARA tanısı konulan tüm hastalar primer ve sekonder penisilin profi laksisi alıyordu. Tedavi sonrası ortalama 3.4±2.7 yıl izlenen karditli hastalardan; MY olan 62 hastanın 20'inde (%32.2) mitral kapak tutulum derecesinde gerileme, 7 (%11.3) hastada ise tam düzelme gözlendi. AY olan 35 hastanın 7'inde (%20) AY derecesinde gerileme, 10 (%28.6) hastada ise tam düzelme gözlendi.Sonuç: ARA, akut dönemde tedavi edilip primer ve sekonder penisillin profi laksisi yapıldığında mekanik kapak replasmanı gibi önemli morbiditelerin önlenebildiği bir hastalıktır.Anahtar Sözcükler: Romatizmal ateş, Benzatin penisilin, Antibiyotik profi laksisi ABSTRACT Objective: To show the effectiveness of primary and secondary penicillin prophylaxis for acute rheumatic fever patients. Material and Methods:Patients diagnosed with acute rheumatic fever and followed-up at the pediatric cardiology department between November 1997 and August 2012 were studied retrospectively regarding their demographic and clinical characteristics. Results:We included 71 patients (53.5% female and 46.5% male) diagnosed with Acute Rheumatic Fever (ARF) in this study. The mean age of the patients was 10.8±3.0 years. Carditis was the most frequent clinical fi nding and was seen in 90%. The mitral valve was affected in 96% of the carditis patients. The most frequent valvular involvement was combined mitral regurgitation (MR) and aortic regurgitation (AR), seen in 33 (51.6%) of the patients. Primary and secondary penicillin prophylaxis were provided to all patients. The patients with carditis were followed up for a mean duration of 3.4±2.7 years after the diagnosis. The degree of MR was decreased in 20 (32.2%) cases and the MR completely recovered in 7 (11.3%) cases among the 62 patients with MR. The degree of AR decreased in 7 (20%) cases and the AR recovered completely in 10 (28.6%%) cases among the 35 AR patients. Conclusion:The important morbidity caused by ARF can be prevented with the primary penicillin prophylaxis in the acute phase and secondary penicillin prophylaxis afterwards.
El estilo de pintura renacentista refleja todo tipo de principios, enfoques y comprensión del diseño plástico como un carácter estándar de las características históricas, políticas, religiosas, culturales y económicas de esa época. Al deshacerse de un tipo de perspectiva estricta y puramente centrada en la iglesia, los artistas del Renacimiento prácticamente comienzan a mirar la naturaleza, la ciencia, la literatura, la filosofía y, obviamente, la anatomía humana en un conjunto de mentes diferente. Una descripción razonable, equilibrada, científica y lógica de la vida humana lleva a los pintores a centrarse más en las partes del cuerpo humano. Aparte de los rostros de las figuras, las manos se convierten en partes esenciales del cuerpo para sujetar un espejo al universo interior de las figuras. Se incluyeron dentro del programa iconográfico tal que reflejaban las emociones y contemplaciones de la figura, y la conexión entre figuras y objetos. En este estudio, un examen a través de varias pinturas renacentistas arrojará luz sobre cómo los pintores como Giotto, Da Vinci, Michelangelo, Francesca y más utilizaron la representación de las manos como un aparato y como un lenguaje de sus propósitos creativos. Así, se manifestará una iconografía de manos en la pintura renacentista. Renaissance style of painting reflects all types of plastic design principles, approaches and understanding as a standard character of historical, political, religious, cultural, and economic features of that era. Disposing of unadulterated strict and church focused kind of perspective, Renaissance artists practically start to look on nature, science, literature, philosophy, and obviously human anatomy in a different set of minds. Reasonable, balanced, scientific and logical portrayal of human life leads painters to focus closer on pieces of the human body. Aside from the faces of the figures, hands become essential body parts to hold a mirror to the interior universe of the figures. They were included within the iconographic program such mirrored the emotions and contemplations of the figure, and the connection among figures and objects. In this study, an examination through various Renaissance paintings will shed light on how the painters like Giotto, Da Vinci, Michelangelo, Francesca, and more utilized the portrayal of hands as an apparatus and as a language of their creative purposes. Thusly, an iconography of hands in Renaissance painting will be manifested.
