Access to adequate water and sanitation is limited in informal settlements, contributing to girls’ challenges managing menstruation at school, especially when they cannot access materials to absorb menstrual blood and appropriate facilities for hygiene. This study documents differences between girls’ experience of menstruation at public schools (where the Kenyan government provides menstrual pads) and private schools (where pads are not provided) in two informal settlements of Nairobi, Kenya. Results showed that supply chains to public schools were not reliable, and equitable pad provision was not assured. Girls in private schools struggled to access pads because they were not provided. Sanitation facilities were physically available, but Muslim girls were unable to practice ablution due to the design of toilets in our study schools. Girls experienced fear and anxiety due to harassment from male peers and had incomplete information about menstruation from teachers. Findings suggest that practitioners and policy-makers should acknowledge the diversity of school populations and monitor programs to ensure efforts do not contribute to inequity.
Abuse and neglect among older adults impact everyone and are recognized internationally as significant and growing public health issues. A systematic review of reviews was conducted to identify effective strategies and approaches for preventing abuse and neglect among older adults. Eligible reviews were systematic or meta-analyses; focused on the older population as reported in the publications; reviewed prevention interventions; included relevant violence and abuse outcomes; written in English; and published in a peer-reviewed journal between January 2000 and May 2020. Eleven unique reviews (12 publications) met the eligibility criteria, including one metaanalysis. Included reviews mainly focused on general abuse directed toward older adults; and educational interventions for professional and paraprofessional caregivers, multidisciplinary teams of health care and legal professionals, and families. Interventions were implemented in a variety of community and institutional settings and addressed primary, secondary, and tertiary prevention. The reviews indicated weak or insufficient evidence of effectiveness in preventing or reducing abuse, yet several promising practices were identified. Future research is needed to evaluate emerging and promising strategies and approaches to prevent abuse among older adults. Effective interventions are also needed to prevent or reduce abuse and neglect among older adults.
Public health–related decisions are influenced by a variety of actors operating on local to global levels, including community leaders, educators, nongovernment organizations, government officials, donors, and researchers, many of whom may lack formal public health training. The provision of public health instruction to interdisciplinary professionals has the potential to strengthen the capacity of all stakeholders to make informed, evidenced-based decisions about health policies and programs. The use of online learning is emerging as a promising means of providing public health training, particularly among those living in geographically disparate areas and from multidisciplinary backgrounds. This article describes an online course created to teach participants in stakeholder teams from 14 low- and middle-income countries how to design and conduct qualitative research to understand girls’ challenges managing menstruation at school. The goal of the course was to strengthen each country team’s ability to conduct research by building the capacity of the members. Thus, completion of the course by all team members was an objective, but less of a focus than assuring that each team as a collective was gaining public health insights and working together to make informed decisions about their research goals. This course led to benefits beyond capacity strengthening, including the formation of a broader community of learning and practice that extended beyond country boundaries. We recommend embedding training opportunities for multidisciplinary stakeholders into research endeavors given the potential for positive effects on individual participants and overall policy decisions to improve community health and provide lessons learned for doing so.
