Introduction: There are important parameters in understanding the severity of COVID-19 infection. We aimed to observe the role of systemic inflammatory index and neutrophil lymphocyte ratios in Covid-19 infections and the changes seen during the infection process. Method: This retrospective single-center study was conducted between April 2020 and May 2020. Clinical, demographic and laboratory data of patients with COVID-19 were analyzed. All COVID-19 case confirmation made with reverse transcription polymerase chain reaction. Neutrophil/lymphocyte ratio and SII change and relationship were examined. Results: 140 patients were included in our study and 50 of them were female. There was no statistically significant difference in SII according to gender (p>0.05) but there was a difference in SII between age categories (p=0.009). A statistically significant difference was found (p=0.003) regardless of the presence of comorbidities. Tobacco users had a significantly lower SII (p=0.028). A statistically significant difference was found between SII and Covid Severity Index (p=0.020). Giriş Hem sağlık hizmet sunucusu hem de çalışma ortamı olan sağlık kurumlarında İş Sağlığı ve Güvenliği yüksek riskleri nedeniyle önemlidir. Çalışmamızdaki amaç, sağlık sektöründe çalışanların ‘Temel İş Sağlığı ve Güvenliği’ konusunda bilgilerini araştırmak ve bu konuda yapılan eğitimin bilgi birikimlerine ne kadar etkili olduğunu incelemektir. Metod: 01.06.2020 ve 01.12.2020 tarihleri arasında devlet hastanesinde, öncesinde Temel İş Sağlığı ve Güvenliği eğitimi almamış 65 farklı meslek grubundan oluşan sağlık sektörü çalışanlarına verilen Temel İş Sağlığı ve Güvenliği eğitimi, güncel kaynaklarla hazırlanan 10 soru/100 puanlık eğitim öncesi ve sonrası test ile değerlendirilmiştir. 7 soru/70 puan başarılı kabul edilmiştir. Verilerin yüzdesel değişimleri tablolama programı ile istatistiksel formüller üzerinden hesaplanmıştır. Bulgular: Çalışmaya 65 farklı meslek grubundan, 392’si erkek ve 315’i kadın, 203’i hemşire, 82’i temizlik personeli, 69’u ebe, 53’ü hekim olmak üzere toplam 707 çalışan katılmıştır. Ön-test sonuçlarında 70 puan altında kalanların sayısı 391, 0 alanların sayısı 77, 100 alanların sayısı 5’tir. Son test sonuçlarında puan ortalaması 80,19, başarısız olan çalışan sayısı 0 olup ortalama doğru cevap verilen soru sayısı 8,01, yanlış soru sayısı 1,95 ve boş sayısı 0,04 olarak saptanmıştır. Mesleklere göre en yüksek ortalama puan; ön-test sonuçlarında sağlık teknikerlerinde, son-test sonuçlarında ise hekimlerde olmuştur. Sonuç: Sağlık sektörü çalışanlarının İSG bilgilerinin yetersiz olduğu ancak verilen eğitimlerle bu sorunun aşılacağı görülmüştür. Tüm çalışanların dahil edildiği eğitimler ile İSG konusundaki risklerin azaltılması, hem çalışanlara hem de hastane ortamında sağlık hizmetinin sürdürebilirliğine önemli katkılar sağlayacaktır.

Purpose Gastrointestinal system bleeding (GIB) is a common and highly morbid condition with high mortality rates frequently observed in emergency departments. Several scoring systems have been used to predict mortality and rebleeding in these cases. Among these scoring systems, Rockall Score (RS) and Forrest classification (FS) are commonly used. In our study, we aimed to evaluate the predictive power of RS for mortality, rebleeding, and the need for blood transfusion, and to provide benefits to clinicians. Methods The study was designed as a retrospective and single-center study, conducted between January 1, 2017, and January 1, 2018. Demographic data, RS and FS scores, clinical outcomes (mortality, rebleeding, blood transfusion requirement, and final outcomes) of the cases were evaluated. Results Of the 109 patients in our study, 60.6% were male, and the mean age was 67.83 ± 16.75 years. The most common presenting complaint was black stool, and the most frequently detected finding was melena. The most common endoscopic diagnosis was erosive gastritis (24.8%). The use of acetylsalicylic acid and anticoagulants was significantly higher in cases of lower gastrointestinal bleeding. There was a negative correlation between the RS score and hemoglobin level of the patients (r=-0.537; p < 0.001), while a positive linear correlation was observed between RS and mortality (r = 0.352; p < 0.001), rebleeding (r = 0.381; p < 0.001), and blood transfusion requirement (r = 0.489; p < 0.001). Additionally, there was a positive linear correlation between the amount of blood product given to the patient and RS (r = 0.432; p < 0.001). The discriminatory power of RS for predicting mortality, rebleeding, and blood transfusion was evaluated, and our results showed an AUC of 0.802 (95% CI = 0.684–0.921; p < 0.001) for mortality, an AUC of 0.764 (95% CI = 0.641–0.886; p < 0.001) for rebleeding, and an AUC of 0.815 (95% CI = 0.731–0.899; p < 0.001) for blood transfusion requirement. Conclusions RS provides risk stratification in cases of GIB and aids in early treatment and prognosis prediction for high-risk patients. Furthermore, it has a high predictive power in determining rebleeding, mortality, and the need for blood transfusion. We believe that RS can be beneficial for clinicians in the management, prognosis, and mortality risk assessment of GIB cases, particularly in clinics such as emergency departments where a large number of patients need to be evaluated and treatment and discharge decisions are made within a short period of time.

Introduction: It is known that children with neurodevelopmental diseases have frequent traumas like falling and apply to emergency department which significantly affects patient and family. Our aim is to examine the families' view and the problems encountered in emergency services by examining their answers to questionnaire. Method: This study was designed retrospectively. Patients registered in Department of Autism, Mental Special Needs and Rare Diseases as families with children who have neurodevelopmental diseases included in the study. 324 families’ survey with 12 open-ended questions evaluated in terms of distribution and frequencies with descriptive statistical methods. Results: By analyzing the data obtained the findings of the research were gathered under 12 main themes and 80 sub-themes. 313 (89.9%) were diagnosed with Down Syndrome, 9 (2.5%) with Angelman Syndrome, 7 (2.02%) with intellectual disability, and 5 (1.4%) with Autism. 34.2% of the children were 0-3 years old, 28.7% were 3-6 years old, 20.9% were 6-10 years old. Emergency services were used in "all cases where urgent intervention is required" (70%), and in cases where “an outpatient appointment couldn’t made" (10.8%) Conclusion: It would be beneficial to examine children with neurodevelopmental diseases in an understanding and patient manner, using a plain and understandable language, in a quiet environment in emergency room. To solve these patient’s problems, awareness training of healthcare professionals, having separate examination and observation room, ensuring accessibility and arranging the room with necessary medical supplies and equipment should be made.
