Aim and objective To investigate the effectiveness of music interventions on physiological and psychological stress response in intensive care unit (ICU) patients. Background ICU patients are exposed to several physiological and psychological stressors during their treatments. Music interventions can be implemented to control these negative effects. Design Systematic review and meta‐analysis conducted according to PRISMA Guidelines. Method Seven electronic literature databases, reference lists of similar reviews, grey literature and the Clinical Trials Registry were searched for potential studies up to 30 January 2022. Studies were expected to meet PICOS inclusion criteria. Two reviewers independently assessed the risk of bias of the included studies by using the Cochrane Collaboration tool. Overall, meta‐analysis and subgroup analyses (comparator group and music session frequency) were performed using RevMan 5.4. Meta‐analysis was conducted when data were available; otherwise, a narrative description was provided. Results Twenty‐five articles were included in this meta‐analysis. Across all studies, regardless of the comparator, music was found to significantly reduce anxiety scores with an SMD of 1.81 ([95% CI: −3.09,‐0.53]; p = .006). Music was found to significantly reduce anxiety scores with an SMD of 1.97 (95% CI: −3.66, −0.28) compared to standard care (p = .02), but there was no significant change in anxiety scores in comparison with the noise cancellation group (p > 0.05). It was found that multiple music sessions reduced the anxiety level better than a single music session. Music had no effect the systolic/diastolic blood pressure, respiratory or heart rate level in both subgroup and general analyzes concerning the physiological stress (p > 0.05). Conclusions Music interventions involving multiple sessions can be used as a nursing intervention to control the anxiety levels of ICU patients. Relevance to the clinical practice Using music to reduce anxiety and stress levels may reduce the pharmacological need (for sedative or antipsychotic medications) and the risk of associated side effects in ICU patients.
Amaç: Bu araştırmanın amacı geriatri kliniğinde tedavi gören 65 yaş ve üzeri yaşlı bireylerin günlük yaşam aktivite durumu ile düşme riski arasındaki ilişkiyi incelemektir. Gereç ve Yöntem: Bu çalışmanın yöntemi tanımlayıcı, kesitsel ve ilişkisel olup örneklemini dâhil edilme kriterlerine uyan ve bir geriatri kliniğinde tedavi gören 65 yaş üzerindeki 96 yaşlı birey oluşturmuştur. Veri toplama aracı olarak literatüre dayanarak araştırmacılar tarafından oluşturulmuş 11 soruluk “Birey Tanılama Formu”, “Katz Günlük Yaşam Aktiviteleri (GYA) Ölçeği” ve “Mors Düşme Riski Ölçeği” kullanılmıştır. Bulgular: Katılımcıların yaş ortalaması 75,17±7,06, %64,6 kadın, %47,9’u fazla kilolu, tümünde en az bir kronik hastalık ve düzenli ilaç kullanımı mevcuttur. Yaşlı bireylerin KATZ GYA ölçeği puan ortalaması 2,14±2,36; %58,3’ü bağımlıdır. Morse düşme ölçeği puan ortalaması 49,11±24,80 ve %34,4’ü yüksek risk grubundadır. Alzheimer hastalığı, hipokampus, serebral korteks ve subkortikal alanlarda senil plaklar ve nörofibriler düğümlerin varlığı ile karakterize en sık görülen nörodejeneratif hastalıktır. Hastalığın erken aşamaları klasik olarak bellekteki zayıflama ile ilişkilidir. Hastalığın erken evresinde bireyler kelime bulmakta, yeni insanların isimlerini hatırlamakta ve sosyal/iş görevlerinde zorluk yaşamaktadırlar. Beraberinde son günlerde okuduğu yazıyı unutma ve nesneleri yanlış yerleştirme gibi sorunlar görülebilmektedir. Hastalık ilerledikçe orta evrede önemli olayları veya kişisel anıları unutmak, sosyal geri çekilme, adres veya telefon numaralarını hatırlayamama, zaman ve yer oryantasyonunda bozulma, mevsime/ortama uygun kıyafet seçememe, uyku düzeni değişiklikleri, gezinme veya kaybolma riski, kişilik veya davranış değişiklikleri (şüphe, dürtüsellik, sanrılar ve tekrarlayan davranışlar) görülmektedir. Alzheimer hastalığının neden olduğu bu sorunların sona erdirilmesi için erken tanı, tedavi ve özellikle de önlenmesine yönelik çeşitli yaklaşımlara gereksinim vardır. Teknolojinin de ilerlemesiyle dünyada ve ülkemizde çalışmaların artmasına rağmen hastalığın önlenmesi ve kesin tedavisi henüz mümkün olmamaktadır. Ancak Alzheimer hastalığına sahip yaşlı bireylerin günlük yaşam aktivitelerine katılımı ve yaşam kalitesinin artırılabilmesi için ilaç tedavisinin yanı sıra özellikle hastalığın belirtilerini azaltmakta ilaç dışı yöntemler de uygulanmaktadır. Bu derleme, farmakolojik tedavinin yanı sıra Alzheimer hastalarının semptomlarını azaltmak ve yaşam kalitesini artırmak için kullanılabilecek geleneksel ve destekleyici yöntemlerin hemşireler ve diğer sağlık profesyonellerine yol göstermesini amaçlamaktadır.
