Introdução: Algumas neoplasias malignas de mama expressam a proteína HER2. Estes são denominados câncer de mama HER2+, esta proteína está envolvida com a proliferação celular. Esses cânceres geralmente crescem e se espalham rápido, no entanto, respondem melhor ao tratamento com fármacos que têm como escopo a proteína HER2. Trastuzumab deruxtecan é um anticorpo conjugado direcionado às neoplasias malignas HER2-positivas, atua sobre os tumores que expressam a HER2, inibindo a topoisomerase I do DNA. Este fármaco vem se mostrando eficaz no tratamento do câncer de mama que expressa HER2. Objetivo: Revisar o uso da imunoterapia no câncer de mama, com enfoque na medicação Trastuzumab Deruxtecan. Material e métodos: Foi utilizada as bases eletrônicas do PubMed, com a estratégia de busca: \"Trastuzumab Deruxtecan\" AND \"Breast cancer\". Foram incluídos artigos publicados entre 2017 e 2022, com textos completos e condizentes com o objetivo do trabalho; em inglês, espanhol e português. Sendo encontrados 71 artigos, sendo mantidos 5 artigos por atenderem aos critérios estabelecidos. Resultados: Os fatores de crescimento epitelial (EGFs) se ligam ao HER2, ativando e logo depois o dímero fosforila a si mesmo levando à sinalização RAS/MEK/ERK que aumenta a sobrevivência celular, enquanto a via PI3K/AKT/mTOR estimula a proliferação celular. O anticorpo monoclonal anti-HER2 IgG1, trastuzumab, tem a mesma sequência terminal de aminoácidos dos agonistas (EPGs), ligando-se no HER2 como um antagonista e bloqueando a sinalização. Logo, o bloqueio acaba inibindo a proliferação celular do câncer de mama e das metástases cerebrais. A dose padrão do trastuzumabe deruxtecan é de 5,4 mg/kg, tem um T1/2 de 7 dias, sendo metabolizado em grande parte pela CYP3A4, apresentando uma depuração de 0,42 L/dia. Conclusão: Os estudos demonstraram melhor prognóstico para os pacientes com câncer de mama que expressam a proteína, que fizeram o uso do Transtuzumb Deruxtecan como o estudo realizado com 184 pacientes. A duração média da resposta foi de 14,8 meses. A mediana da duração da sobrevida livre de progressão cancerígena foi de 16,4 meses. Entre os 24 pacientes inscritos com metástases cerebrais, estima-se uma sobrevida global de 93,9%.
Introdução: A Naegleria fowleri é um protozoário de vida livre, flagelado e termofílo, popularmente conhecido como ameba "comedora de cérebro", é o agente etiológico de uma doença da meningoencefalite amebiana primária (MAP). Uma doença que normalmente afeta jovens adultos, e tem 95-99% de taxa de mortalidade. Objetivo: Revisar a MAP causada por N. fowleri. Material e métodos: Foi realizada uma revisão bibliográfica na base de dados BVS (Biblioteca Virtual em Saúde) e PUBMED com a seguinte combinação de descritores: "Central Nervous System Protozoal Infections" AND "Naegleria fowleri”. Foi definido os seguintes critérios de inclusão: textos condizentes com os objetivos e tema; em português e inglês; e publicados nos últimos 5 anos. A busca retornou 19 artigos, após a aplicação dos critérios de inclusão, foram mantidos e analisados 11 artigos. Resultados: Os artigos revisados enfatizam que o N. fowleri é morfologicamente dividido em: cisto, trofozoíto e flagelados, sendo a trofozoíta. O ciclo de vida do geralmente acontece em ambientes aquáticos, quentes e ricos em nutrientes, eles podem ser encontrados em solos, piscinas e esgotos. No entanto, podem se tornar patogênicos em seres humanos, ao penetrar na mucosa nasal e migrar para o cérebro através do nervo olfatório, então, alcançam a membrana aracnóide, espalhandose pelo sistema nervoso central, levando o paciente ao óbito. O período de incubação ocorre entre 2 a 3 dias ou 7 a 15 dias. Os sintomas são caracterizados por encefalite, cefaleia bifrontal, febre, náuseas e êmese. Às vezes, no início da progressão da doença, parosmia e ageusia ocorrem à medida que os trofozoítos danificam o sistema olfatório. Embora alguns medicamentos empregados no tratamento de AMP como: Anfotericina B, Miltefosina, Rifampicina, mostrem resultados positivos, é difícil afirmar se a doença está sendo tratada corretamente usando uma determinada combinação deles. Conclusão: A MAP é uma doença emergente e infelizmente negligenciada, faz-se necessário mais estudos para melhor proceder com o diagnóstico e desenvolver fármacos mais eficientes e específicos para esta doença.
