Auf der Grundlage eines kognitiv-verhaltenstheoretischen Modells soil die Bedeutung psychologischer Variablen für die Entwicklung von Depression bei chronischem Schmerz untersucht werden. Die hier ausgewählten Variablen sind tägliche Stressoren und die von Kuhl entwickelte Handlungsorientierung (HAKEMP). Es wurden 183 Patienten mit chronischem Schmerz zu Beginn ihres stationären Aufenthalts in einer Spezialklinik untersucht. Von den Schmerzpatienten hatten 22% erhöhte Depressionswerte (Beck-Depressions-In-ventar 18). Depression ist unabhängig von der Erkrankungsdauer und steht mit keinem der Schmerzparameter und dem Behandlungserfolg in Beziehung. Von der Varianz der Depressionswerte können 48% durch die psychologischen Variablen vorausgesagt werden; keine der schmerzbezogenen Variablen hat einen signifikanten Einfluβ auf die Depressionswerte. Aus den Ergebnissen wird der Schluβ gezogen, daβ kognitive Mechanismen bei der Entwicklung depressiver Symptome auch in der Gruppe chronischer Schmerzpatienten eine wichtige Rolle spielen.
