Die kritische Extremitätenischämie (KE) beschreibt einen Grad der Minderdurchblutung, der für den Extremitätenerhalt eine unmittelbare Gefährdung darstellt. Die Vitalität der beteiligten Strukturen ist in diesem Zustand zwar noch reversibel bedroht, aber sie stehen unmittelbar vor dem irreversiblen Gewebsuntergang. Aufgrund der Tatsache, dass die Muskulatur mit etwa 6 Stunden die geringste ischämische Toleranzzeit von allen beteiligten Geweben aufweist, ist die Zeit bis zur irreversiblen Gewebeschädigung durch diese Zeitspanne charakterisiert [2, 46]. Im Hinblick auf die Akuität der Ischämieentwicklung werden die akute von der chronischen Extremtätenischämie unterschieden [31]. Die akute kritische Extremitätenischämie (AKE) beschreibt eine schnell fortschreitende oder aber plötzlich eintretende Verminderung der Extremitätenperfusion, die entweder mit einer Verschlechterung bereits bestehender Ischämiesymptome einhergeht, oder aber durch schlagartiges Einsetzen vorher nicht bestandener Ischämiesymptome gekennzeichnet ist. Das Fortschreiten von Claudicatio zu Ruheschmerz bis zum ischämischen Gewebedefekt (i. e. ulcus arteriosum) kann schrittweise erfolgen und stellt in der Regel den Endpunkt fortschreitender arterieller Verschlussprozesse dar. Eine akute Verschlechterung der Gehleistung von beispielsweise 1000 m auf 120 m kann zwar die Folge eines akuten Gefäßverschlusses sein, jedoch ist diese Symptomatik mit einer AKE nicht gemeint [32]. Das Bild der chronischen kritischen Extremitätenischämie (CKE) dagegen ist dem Kliniker zwar in der Regel geläufig. Der Versuch, diesen Zustand einheitlich und präzise zu definieren, ist jedoch schwierig. Klinisch handelt es sich um Patienten mit typischem ischämischen Ruheschmerz oder mit einem ischämischen Gewebedefekt.
