Differenzialdruckventile haben bei Patienten mit einem Normaldruckhydrozephalus den Nachteil, dass sie sich bei ruckartigem Aufstehen schiagartig öffnen bzw. zu lange offen bleiben und dabei elnen Sag auf den Liquorraum des bereits atrophisch vorgeschadigten Hirns ausUben. Kännen diese Nachteile und insbesondere Uberdrainagekomplikationen durch hydrostatische Ventile verringert bzw. behoben werden? Bei insgesamt 128 Patienten mit einem Normaldruckhydrozephalus wird das Miethke-DuaI-Switch-Ventil implantiert. AIle 128 Patienten nahmen an der Nachuntersuchung durchschnittlich 7 Monate postoperativ innerhalb dieser Multizenterstudie teil. Die Evaluation des Krankheitsverlaufes nach Shuntimplantation erbrachte 63% gute bis sehr gute Ergebnisse, 16% befriedigende und 21 % schlechte. Dabei korrelierte das Outcome mit der prã-operativen Schwere der Erkrankung, graduiert nach der KieferScale. lnfektionsrate, Shuntinsuffizienz, Ventildislokation undokklusion sowie die Auswahl der Ventildruckstufen werden diskutiert. Das Arbeitsprinzip dieses hydrostatischen Ventils wird vorgestellt. Mit einer Uberdrainagerate von 2% und einer shuntassoziierten Letalität von 0% liegen die Komplikationen beim Miethke-Dual-Switch-Ventil deutlich unter den internationalen Zahlenangaben. Der Krankheitsverlauf bei Patienten mit einem Normaldruckhydrozephalus wi rd vom Stadium der Erkrankung zum Zeitpunkt der Therapie und von der Art des implantierten Ventils bestimmt. Trotz der noch weltweit geringen Erfahrungen mit dem Miethke-Dual-Switch-Ventil soliten die Vorteile dieser hydrostatischen Ventilart bei Patienten mit einern Normaldruckhydrozephalus hervorgehoben werden.In patients with normal pressure hydrocephalus (NPH) the conventional differential pressure valve has the disadvantage of opening abruptly when the patient moves into an upright position. A strong suction at the already atrophic brain could therefore result. Can this disadvantage and especially the complication of overdrainage be reduced or solved by a hydrostatic valve? In 128 patients diagnosed for NPH we implanted a Miethke DualSwitch valve. All patients were reevaluated 7 months after surgical treatment in our multicentre study. The evaluation of the course of disease showed in 63% of cases an excellent outcome. in 16% a satisfactory and in 21% of the patients a bad outcome. The patients, outcome correlated with the preoperative severity of the disease scored using the Kiefer scale. Complications as well as the valve pressure levels are discussed in the paper. We also present the technical principles of this new valve technology. With a overdrainage rate of 2% and a shunt-related lethality of0%the rate of complications of the Miethke Dual-Switch valve is clearly lower than started in international literature. The clinical course of patients suffering from NPH is influenced by the stage of the disease, the beginning of therapy and the implanted valve type.
