ZusammenfassungLangzeitstudien weisen auf eine wesentlich ungünstigere Prognose von Schizophrenien hin, die sich bereits in der Kindheit bzw. Adoleszenz manifestieren. Die Vollremissionsrate liegt mit 25 % um die Hälfte niedriger als bei Schizophrenien, die im Erwachsenenalter beginnen. In einer eigenen Untersuchung, die sich über einen Katamnesezeitraum von 1957 bis 1993 erstreckte und in die alle Patienten (n = 71) eingingen, die entweder nur in der Kinder-und Jugendpsychiatrie an der Universität Rostock stationär oder später in einer Erwachsenenpsychiatrie mit der Diagnose Schizophrenie bzw. schizoaffektive Störung behandelt wurden, konnte diese pessimistische Prognose nicht aufrecht erhalten werden. 69 % wiesen einen guten Heilungsverlauf auf. Für einen günstigeren Verlauf sprechen anhand der vorliegenden Ergebnisse das weibliche Geschlecht, eine Erstmanifestation jenseits des 14. Lebensjahres, weniger Erkrankungsepisoden im Kindes-und Jugendalter, eine höhere Intelligenz und der Status Erstgeborener in der Geschwisterreihe. With a rate of 25 %, the complete remission is half the rate of psychoses beginning in adulthood. Our own results do not confirm this overall pessimistic prognosis. In our catamnestic study covering a period from 1957 to 1993, we included 71 patients who had been diagnosed in the department of child and adolescent neuropsychiatry at the University of Rostock with schizophrenia or affective psychotic disorder and had been treated either just in that hospital or later on in the adult's psychiatric hospital. As much as 69 % present a good course of disease. The results suggest a better prognosis if: patients are of female gender, schizophrenia did not appear before the age of 14, less episodes of psychoses in childhood and adolescence have occurred, if the patients showed higher intelligence and were first born children.
