In der Literatur wird die Bestimmung des carcinoembryonalen Antigens (CEA) im Plasma und Urin zur Tumorsuche beim Blasenkarzinompatienten mit einer Erfolgsquote von 40–80% angegeben. Patienten mit radikaler Zystektomie wegen eines infiltrierenden Blasenkarzinoms und intestinaler Harnableitung in Form eines Colon-Conduits oder einer Uretero-Sigmoideostomie wiesen in der postoperativen Nachkontrolle im hohen Prozentsatz erhöhte CEA-Werte auf die zur Metastasensuche Anlaß gaben. In alien Fallen ließ sich keine Tumoraktivität nachweisen. Deswegen wurde eine prospektive Studie mit 54 zystektomierten Blasenkarzinompatienten durchgeführt, die eine intestinale Harnableitung erhalten hatten. Als Kontrollgruppe dienten 49 zystektomierte Non-Tumor-Patienten. Es wurde in Serienintervallen das CEA sowohl im Plasma als auch im Urin bestimmt. In über 80% der Non-Tumor-Patienten waren die CEA-Werte erhöht; das Urin-CEA war in über 90% positiv (m = 2000 ng/ml). Nur 3 der 5 Patienten mit nachgewiesenen Tumormetastasen zeigten überhaupt CEA-Erhöhungen.
