Les médicaments génériques ont une place croissante dans l'arsenal thérapeutique vétérinaire. Une ligne directrice européenne définit les modalités de constitution et d'évaluation du dossier d'autorisation de mise sur le marché de ces médicaments. L'exposé des limites scientifiques permettra aux vétérinaires de garder une analyse critique sur l'usage de ces médicaments dans certaines situations particulières telles que les traitements collectifs, l'antibiothérapie ou la substitution.Mots-clés : médicament générique, bioéquivalence, vétérinaire. An European guideline defines the methods of constitution and evaluation for marketing authorization application. SUMMARYLes médicaments génériques appartiennent à l'arsenal théra-peutique vétérinaire, et leur nombre croît année après année. Après un rappel de définitions et des outils réglementaires permettant leur évaluation, une mise au point sur les limites scientifiques de l'autorisation de mise sur le marché (AMM) de ce type de médicament est proposée. DÉFINITION D'UN MÉDICAMENT GÉNÉRIQUEDans la directive sur la conduite des études de bioéquivalence des médicaments vétérinaires (European Medicine Agency (EMA), 2011), se trouvent les bases légales, les définitions et tous les éléments réglementaires permettant d'aborder l'éva-luation des médicaments génériques vétérinaires.Un médicament générique contient la même quantité du (des) même(s) principe(s) actif(s) que le médicament de réfé-rence sous la même forme pharmaceutique; cependant, il peut s'agir de sels différents et/ou de différences portant sur la composition excipiendaire (colorants, arômes,…). Le médicament générique est administré suivant les mêmes modalités que le princeps : dose, voie, durée… Par définition, un médicament générique doit répondre à des critères de bioéquivalence par rapport à un produit de référence. Ainsi, des produits sont bioéquivalents si leur biodisponibilité après administration à une même dose selon une même voie est suffisamment similaire pour que leurs effets et leur sécurité soient considérés comme étant essentiellement similaires.À partir des principes des relations pharmacocinétique / pharmacodynamie (PK/PD), il est alors admis qu'à une dose donnée, on observe un effet (thérapeutique et/ou toxique) donné. Ainsi, après administration à un organisme, dés lors que les deux formulations (générique et princeps) assurent des expositions très voisines au(x) principe(s) actif(s), les effets obtenus sont considérés comme similaires (mais pas identiques).Les excipients participent à l'absorption, la rémanence voire à...
L'utilisation des agents anticancéreux en pratique vétérinaire est problématique en raison des difficultés d'approvisionnement en médicaments cytotoxiques, mais aussi des risques liés à l'utilisation de telles substances pour le vétérinaire, son personnel, le propriétaire de l'animal et son entourage, ainsi que pour l'environnement. En effet, la plupart de ces molécules anticancéreuses sont cancérogènes, mutagènes et/ou tératogènes.Afin d'éliminer les réserves formulées par les autorités de tutelle sur la capacité des vétérinaires praticiens à utiliser les agents anticancéreux cytotoxiques, il est indispensable de mettre en place une formation initiale de qualité, suivie d'une formation continue rigoureuse et d'une responsabilisation des vétérinaires.Il convient également de s'interroger sur l'éthique et la justification de certains choix thérapeutiques en cancérologie vétérinaire, en tenant compte de critères d'efficacité, du rapport bénéfice/risque, de critères économiques, et aussi et surtout du risque pour la société et l'environnement.
