Schöpferische Atmosphäre, Entwicklung neuer Gedanken, Kooperation mit Nachbargebieten oder mit Bereichen aus den Grundlagenwissenschaften und beste klinische Führung sind Voraussetzungen für fruchtbare Forschung. Erfolgreiche chirurgische Forschung erfordert aber auch den adäquaten zeitlichen, räumlichen und finanziellen Rahmen neben der groûen zeitlichen Belastung, die die Tätigkeit im operativen Gebiet mit sich bringt. Je nach Problemstellung sind ganz unterschiedliche Binnenstrukturen und Organisationsformen notwendig [1,11]. Das pathophysiologische Experiment ± meist als Tiermodell ± setzt andere Strukturen voraus als zell-oder molekularbiologische Forschung. Klinimetrie braucht differente Organisationsabläufe ganz abgesehen von den Grundvoraussetzungen des ¹good clinical practiceª (GCP). Beispielhaft für klinische Forschung sind die von W. Lorenz beschriebenen Arbeitsmodelle der theoretischen Chirurgie [6]. Klassische StrukturenLeistung und Ausstrahlung der Institute für Chirurgische Forschung bzw. für Experimentelle Chirurgie haben in der Vergangenheit eindrucksvolle Weiterentwicklungen in der pathophysiologischen Kenntnis und klinischen Umsetzung gebracht. Hier ist die Herz-Kreislauf-Forschung in Verbindung mit der Herz-Lungen-Maschine, der Aufbau der Transplantationschirurgie und die Schockforschung im weitesten Sinne zu nennen [7]. Mit der Mikrozirkulationsforschung ist jetzt der Brücken-schlag zur Zellbiologie und Molekularbiologie gelungen [8].Die unter dem Leitthema dieses Heftes des ¹Chirurgª zusammengestellten Beiträge zu multimodalen Kombinationstherapien in der Chirurgischen Onkologie zeichnen neben der klinischen Status-quo-Bestimmung weitere notwendige Zielsetzungen klinischer Studien auf. Die Analyse des neuen klinischen Wissens über onkologische Zusatztherapien aus dem letzten Jahrzehnt ist in ihrem Ergebnis nicht so überzeugend wie allgemein angenommen wird. Eine Konsensuskonferenz des NIH in den USA 1990 zum Dickdarmkrebs brachte zwar einen groûen Sprung vorwärts, zeichnete aber auch die Probleme auf, die nach wie vor zur Lösung anstehen. Der Vergleich der laufenden adjuvanten Studien beim Dickdarmkrebs muû zu der Folgerung führen, daû nur unter Einbezug einer standardisierten qualitätskontrollierten Chirurgie und unter Berücksich-tigung von Kontrollgruppen weitere adjuvante Therapien über-prüft werden können. Paradigmawechsel und neue StrukturenDie neue groûe Herausforderung in der Chirurgie bietet die Molekularbiologie, die mit ihren Methoden auf subcellulärer Ebene für chirurgische Fragestellung genutzt werden muû [5]. Dieser immer wieder als wissenschaftlicher Paradigmawechsel bezeichnete Vorgang erfordert gerade in der Chirurgie neue Organisationsformen für die Forschung. Die klinische Forschungsgruppe oder der Sonderforschungsbereich der DFG sind Modelle. Zuvor müssen aber erst ausreichend Kooperationsebenen zwischen Molekularbiologie und Chirurgie geschaffen werden.
