A method was developed to measure simultaneously the rate constants for glucose influx and glucose efflux, and the Michaelis-Menten constant (KM) and maximal velocity (Vmax) for glucose transport across the blood-brain barrier (BBB) in any selected brain area. Moreover, on the basis of a mathematical model, the local perfusion rate (LPR) and local unidirectional glucose transport rate (LUGTR) are calculated in terms of parameters of the time-activity curves registered over different brain regions; 11C-methyl-D-glucose (CMG) is used as an indicator. The transaxial distribution of activity in the organism is registered using dynamic positron-emission tomography (dPET). The method was used in 4 normal subjects and 50 patients with ischemic brain disease. In normals, the rate constant for CMG efflux was found to be 0.25 +/- 0.04 min-1 in the cortex and 0.12 +/- 0.02 min-1 in white matter. In the cortex, the KM was found to be 6.42 mumol/g and the Vmax was 2.46 mumol/g per minute. The LUGTR ranged from 0.43 to 0.6 mumol/g per minute in the cortex, and from 0.09 to 0.12 mumol/g per minute in white matter. The LPR was calculated to be 0.80-0.98 ml/g per minute for the cortex and 0.2-0.4 ml/g per minute for white matter. In patients with stroke, the ischemic defects appeared to be larger in CMG scans than in computed x-ray tomography (CT) scans. Prolonged reversible ischemic neurological deficit was associated with a significant fall in the LUGTR but no change in the LPR in the corresponding cerebral cortex. Normal LUGTR and significantly decreased LPR were registered in a patient with progressive occlusion of the middle cerebral artery. In a patient with transient ischemic attacks, a slightly reduced LPR and a disproportionally reduced LUGTR were observed before operation. After extra- and intracranial bypass surgery, the LPR became normal, whereas the LUGTR increased but did not achieve normal values.
ZusammenfassungMit Indium-113m und der Gamma Retina V wurden radiokardiographisch die minimalen kardialen Transitzeiten zwischen rechtem Vorhof und Aortenwurzel gemessen. Die Messung der Gesamt-MTTs diente der Ermittlung des Quotienten aus Schlagvolumen zu Segmentvolumen des zwischen rechtem Vorhof und Aortenwurzel liegenden Kreislaufabschnitts.Bei 39 Patienten mit Myokardinsuffizienzen verschiedener Schweregrade wurde mit Hilfe der erwähnten Volumenrelation die Wirkung einer bis zu 5 Jahren dauernden Digitalis-Behandlung beurteilt. Die vorgelegten Daten berechtigen zu folgenden Aussagen:Eine Digitalisierung myokardinsuffizienter Patienten führt zu einer Verbesserung der gestörten zentralen Volumenrelation.Die Verbesserung ist bei belastungsinsuffizienten Patienten so ausgeprägt, daß eine nahezu vollständige Normalisierung möglich ist, die sich auch über einen Beobachtungszeitraum von 5 Jahren als stabil erweist.Die Digitalisierung bereits manifest herzinsuffizienter Patienten führt lediglich zu einer partiellen Verbesserung, die sich darüber hinaus in der Langzeitkontrolle als nicht stabil erweist.Das unterschiedliche Verhalten der Relation von Schlagvolumen zu Segmentvolumen bei latent- und manifest herzinsuffizienten Patienten unter einer langjährigen Digitalistherapie unterstreicht die Bedeutung der Digitalisierung des belastungsinsuffizienten Herzens.
A hemorrhagic infarction which caused left hemiparesis and homonymous hemianopia was demonstrated as a hypodense lesion on the first CT; 2 weeks later it was hyperdense and enhanced after injecting meglumine. Positron emission tomography with [11C]methyl-D-glucose also revealed the ischemic lesion, but also marked decrease of glucose uptake in the visual cortex, which indicated decreased functional activity.
Minimal cardiac transit times (MTTs) are differences between times of appearance of the indicator at consecutive segments of the cardiopulmonary circulation. The MTTs are shown to be directly proportional to the quotient of volume to flow. The relationship of the cardiac MTTs to ventricular ejection fraction was derived. Utilizing this technique the ejection fractions of the left and right ventricles in 72 normal individuals were calculated. Theoretical aspects of measuring cardiac MTTs were related to experience of clinical application and confirmed its usefulness.
ZusammenfassungDie vorliegende Arbeit beschreibt eine Methode zur kontinuierlichen Registrierung globaler und regionaler belastungsinduzierter Änderungen des kardialen und pulmonalen Blutvolumens bei Normalpersonen, Sportlern und latent herzinsuffizienten Patienten. Im Gegensatz zu Normalpersonen und Sportlern steigt das Blutvolumen des Gesamtherzens im Verlauf einer Belastung bei Patienten mit latenter Herzinsuffizienz an. Ebenso signifikant sind die unterschiedlichen BlutvolumenÄnderungen über der linken Lunge bei Normalpersonen und Sportlern einerseits und latent herzinsuffizienten Patienten andererseits. Die Methode ist einfach und sicher; sie erlaubt, die bei gesunden Sportlern und herzkranken Patienten offensichtlich unterschiedliche Adaptation des kardiopulmonalen Systems wÄhrend körperlicher Belastung fortlaufend zu erfassen.
