Las anomalías congénitas de los troncos supraaórticos pueden ser diversas, la arteria subclavia derecha aberrante aparece en el 0,5% 1 .Suelen ser asintomáticos, el 5% tienen síntomas por compresión, más frecuente la disfagia 2 .El angio-TAC o la resonancia magnética se usan para su diagnóstico y planteamiento quirúrgico.Describimos un caso con arteria subclavia derecha aberrante y divertículo de Kommerell en angio-TAC.Paciente de 62 años con tabaquismo, hipercolesterolemia, hipertensión arterial, diabetes, cardiopatía isquémica sin posibilidades de revascularización, poliposis colónica y pancreatitis aguda 2 años atrás.Desde hacía un año tenía dolor torácico inespecífico y disfagia ocasional, sin pérdida de peso. En radiografía de tórax, se vió prominencia de botón aórtico y ensanchamiento mediastínico superior con desplazamiento de luz traqueal. El angio-TAC mostró una arteria subclavia derecha aberrante en la parte posterior del cayado, retroesofágica, con dilatación en el origen de 40 ml, tronco común carotídeo y origen independiente de arteria subclavia izquierda ( fig. 1).El esofagograma mostró desplazamiento del esófago hacia delante y compresión del mismo.
