Immunological studies performed in 12 patients with chronic calcifying pancreatitis show significant increase in serum concentrations of IgA and IgM. Presence of autoantibodies to smooth muscle, mitochondria, antinuclear, gastric parietal cells and IgG was not frequent. Immune complexes were detected in the sera of 11 patients. Normal numbers of B lymphocytes and a significant decrease in T-active and total T lymphocytes were observed in peripheral blood. These results suggest an immunologic participation in chronic calcifying pancreatitis and may explain the persistence and progressive nature of the disease.
There are few reports about magnetic resonance cholangiopancreatography (MRCP) in cystic lesions of the pancreas (KLP). For this reason, we have undertaken a prospective study evaluating the diagnostic efficiency of MRCP as compared with ultrasonography. Twenty-four patients with KLP were examined with magnetic resonance imaging (MRI), and standard and cholangiopancreatography. There were seven cases of cysts associated with acute pancreatitis, 11 patients with KLP and chronic calcifying pancreatitis, five cases of cystic neoplasms of the pancreas, and one polycystic disease of the gland. All cases were first submitted to ultrasonography, which failed to diagnose only a case of cyst associated with chronic pancreatitis depicted by MRCP. We used a GE Signa Horizon 1.5-T system (20 examinations) and a Siemens Magneton Plus 1.5-T machine (four examinations). Eleven patients were operated on. In all cases, it was possible to identify the cysts, the main pancreatic duct and the biliary tree, and verify the relationship of the cyst with neighboring organs. Communication of the cyst with the main pancreatic duct was described in five instances, but we cannot be sure that MRCP would have depicted all cyst-duct communications. The MRI and MRCP images were confirmed by surgery in the 11 operated-on cases. The diagnosis of duct alterations and small pancreatic stones in initial cases of chronic calcifying pancreatitis may be problematic. Clinical findings are very important data to be considered in the differential diagnosis of KLP. Together with the clinical data, MRCP is a very important technique in the diagnostic and therapeutic decision making of KLP. Standard magnetic resonance is advisable as part of the examination in all cases. MRCP is not invasive, is reliable if one knows its limitations, and the patient can return at once to his activities. It allows the analysis of many variables in one examination, contributing to better therapy.
IntroduçãoA proximidade entre a informática e a área médica está cada vez mais presente no cotidiano do ser humano e, quando estreitada, podem propiciar grande benefício àquela área, pois o conhecimento de especialistas é disponibilizado em programas computacionais que são acessados por faculdades, instituições e outros profissionais de saúde [1].Para a melhor compreensão da proposta contida neste artigo, três teorias fundamentais devem ser compreendidas: o complexo QRS, o algoritmo Backpropagation, utilizando redes neurais artificiais, e o algoritmo de Balda.O complexo QRS é um tipo de onda característica do eletrocardiograma (ECG). Ele pode ser usado para diagnosticar anomalias no ciclo cardíaco através de sinais elétricos provenientes do coração. Será demonstrado neste artigo, o princípio básico de funcionamento do ECG, para que se possa entender o porquê do uso de algoritmos de detecção de picos QRS em prol de aperfeiçoamentos tecnológicos para a saúde humana e para aplicações em Redes Neurais.O uso de Redes Neurais Artificias (RNAs) tornou-se bastante difundido no mundo inteiro, não somente devido à correlação criada entre o sistema nervoso humano e as RNAs, como também à alta capacidade de resolução de problemas com melhor desempenho quando comparados a outros algoritmos existentes [2].O emprego do algoritmo Backpropagation com MLP (Multi-layer-perceptron) é convidativo para tratar problemas não linearmente separáveis pois permite supervisionar o aprendizado da rede e obter bons níveis de aproximações dos resultados. O aprendizado supervisionado carrega consigo esse nome pois a entrada e saída desejadas do sistema são fornecidas por um supervisor externo. A cada padrão de entrada submetido à rede se faz uma comparação da resposta desejada com a resposta calculada (que representa uma ação ótima realizada pela rede), ajustando-se os pesos das conexões para minimizar o erro.O algoritmo de Balda baseado em derivadas é um loop simples de programa, capaz de identificar picos do sinal de eletrocardiograma humano. Em uma análise de sistemas lineares, o algoritmo corresponde a um bloco que faz atualizações de seus valores para realizar filtragens específicas, ou seja, é um sistema com memória.A intenção deste artigo é propiciar à comunidade científica e profissional uma visão inicial de como sistemas computacionais inteligentes podem ser usados na área de engenharia biomédica, tanto para aproximar algoritmos de processamento de sinais biomédicos como para a modelagem de processos fisiológicos humanos. Frisamos que a contribuição deste estudo está contida na aplicação, pois foi o único encontrado que utiliza redes neurais artificiais para aproximar o algoritmo de Balda, todavia o principal desígnio do artigo é o fomento de novas ideias e criação de estudos futuros.
