Mentre quest'epoca dall'illuminismo discutibile si approssima al ventunesimo secolo, vi permane una realta critica, che riguarda i maltrattamenti ai bambini. La mole di evidenza a questo riguardo e enorme, e di questa la piu grave e costituita dai danni cerebrali irreversibili. Queste lesioni possono derivare da cause dirette o indirette, dall'ambiente sociale oppressivo e violento e da carenze dello sviluppo psicologico. Inoltre, e difficile per la comunita medica, specialmente quella pediatrica e radiologica, non lasciarsi coinvolgere emotivamente eppure mantenersi informata, pur essendo in posizione chiave per contribuire a gestire il problema di queste violenze. Importante il contributo del dr C. Kempe' (che ha coniato il colorito termine «battered child» o bambino malmenato) insieme al dr Fred Silverman 2 , e susseguente al lavoro pionieristico del dr John Caffey 3 , che ha dato rilievo a questo «morbo», un morbo che solo con grande ritardo si e visto riservare la dovuta attenzwne.Sono stati questi ultimi due medici, radiologi pediatri di non poca fama, a fornire il quadro comprensivo dei dati oggettivi relativi alle lesioni viscerali e scheletriche nei bambini malmenati. 11 dr Caffey ha scritto la drammatica poesia:«Povero bimbo scosso, ingenuo monello scrollato tanto dall'amore quanta dall'odio, la lingua di un altro immobile in silenzio, tu taci storie segrete di violenza colpevole, ma quando ti inondiamo le carni con un jlusso radiante, le ossa ammaccate mandano dei luccicchii veritieri. E lo stira-schiaccia -stira, non lo sbatti -picchi -batti, che ti insanguina le ossa e la dura madre».La componente significativa delle lesioni intracraniche nei maltrattamenti dei bambini e stata illustrata per primo da Caffey 3, che ha successivamente ipotizzato la relazione fra queste lesioni e lo sballottamento della testa del piccolo 4 • La presente relazione trattera le anomalie intracraniche di questo genere, la conseguenza piu grave dei maltrattamenti. lntroduzione L'incidenza del bambino malmenato e in aumento, e arriva nei soli Stati Uniti ad almeno 1,5 milioni di casi regolarmente denunciati, di cui circa la meta con maltrattamenti fisici. Sono a1meno 3.000 i decessi conseguenti, e il trauma cranico costituisce la causa piu frequente di questi; la maggioranza sono neonati e lattanti sotto i due anni di eta 5 • 6 • 7 • 8 • 9 • 10 • 11 • 12 • Delle stime per difetto valutano a oltre cinque milioni il numero effettivo dei bambini malmenati.La relazione significativa fra il ritardo mentale, la para1isi cerebrale infantile (1 0%) e i maltrattamenti ' 2 · 13 , insieme all'elevata mortalita per lesioni cerebrali, impongono che debita attenzione venga riservata al problema dal punto di vista del neuroradiologo pediatra.Bisogna altresi tener presente che persino le teeniche neuroradiologiche piu sofisticate possono solo fornire delle evidenze macroscopiche, spesso indirette, di lesioni cerebrali. Un aforisma comune dice che «un bambino non e un piccolo adulto». Questo si applica molto appropriatamente alla neuroradiologia infantile e allo studio delle anomalie del loro sistema nervosa centrale. La scienza della neuroradiologia costituisce una forza primaria ne! campo della medicina, occupando una posizione preminente e talvolta di guida. L'arte di applicarla ai bambini ha rappresentato una sfida, e negli ultimi due decenni e diventata una forza primaria anche nella medicina pediatrica. Il suo concepimento e stato incerto; la nascita, relativamente inosservata; l'infanzia, difficile; l'adolescenza, eccitate; e la sua maturita e certamente mondiale.Il suo concepimento e stato l'unione difficile fra neurochirurghi che volevano fare di tutto e neuroradiologi esperti nella patologia dell'adulto, che credevano di poterlo fare meglio. Ha visto la luce negli ultimi anni sessanta a Londra, St. Louis, Boston e Toronto, ad esempio, facendo si che alcuni neuroradiologi si occupassero specificamente dello studio dei bambini.Nella sua infanzia, le difficolta principali erano costituite dalla necessita di adattare le tecniche usate per gli adulti o di crearne di nuove, e di arrivare a capire le malattie peculiari ai bambini. Le attrezzature erano state progettate per degli adulti. La pneumoencefalografia e la ventricolografia, spesso eseguite sotto anestesia, erano tediose e macchinose; l'angiografia con puntura brachiale retrograda e diretta alia carotide era spesso imprecisa e difficile; la mielografia con Pantopaque, abbastanza rozza. Si dovevano affilare manualmente gli aghi, le minuscole arterie erano dure ma mobili, e l'introduzione del contrasto era arbitraria, l'iniezione subdurale di aria e Pantopaque era cornune e l'anestesia era lunga e spesso difficile per via del continua posizionamento del paziente. Si doveva lavorare duro per arrivare alle immagini, ma questi lunghi sforzi spesso risultavano in delle vere e proprie «opere d'arte anatomica», con creazioni quali l'autotomografia, la pneumoencefalografia e persino l'angiografia.Nella sua adolescenza, la neuroradiologia pediatrica dei primi anni settanta ha assistito a un rapido succedersi di nuovi sviluppi. L'attrezzatura per la pneumoencefalografia Mimer e stata un capolavoro dell'ingegneria ed e stata adattata con successo ai bambini; si e partiti seriamente con l'angiografia selettiva via catetere per bambini in centri come Boston, Toronto, Oslo, Cincinnati e in Australia, per lo piu limitatamente ai grandi ospedali per l'infanzia con neuroradiologi pediatri novellini. I cateteri si dovevano fare a mano, flangiare e incurvare, e spesso si doveva creare del materiale nuovo per i cateteri 3 French, opiegare le guide, affilare gli aghi, modificare gli iniettori meccanici ed elettrici del contrasto.
