There is no single test to diagnose the etiology of meningitis in children promptly and accurately. The finding of leukocyte aggregation in CSF might be of value as a sensitive adjunctive screening tool for the timely diagnosis of bacterial meningitis, recognizing that it has low specificity and potential practical limitations.
Nevirapine used in single doses to prevent mother-to-child transmission has been shown to be associated with the development of transient resistant mutations. Here we describe the presence of V106M in seven out of 141 South African women (5%) 6 weeks after receiving nevirapine. V106M is a novel resistance mutation found in subtype C viruses exposed to efavirenz. This mutation is thus also induced at a low frequency in subtype C viruses exposed to single dose nevirapine.
A single-tube, single-primer-set reverse transcription-PCR assay was developed for the rapid detection of polioviruses in infected tissue culture fluids and clinical materials. The poliovirus-specific PCR primers are located in the VP1-2A region of the poliovirus genome. They generate a 290-bp product and can be used in duplex reactions with general enterovirus primers. The primers span the region used for genotype determination, so that genotype analysis of wild-type polioviruses can be performed by direct sequencing of the PCR products. Of 125 virus isolates typed as polioviruses by neutralization assays, 125 (100%) were also positive by PCR, and of 38 isolates typed as non-polio enteroviruses by conventional techniques, 38 (100%) were also negative by PCR. The assay described here is rapid, highly sensitive, and specific and has clinical applicability in the diagnosis of poliovirus infections.
The genetic relationships between type 1 polioviruses circulating in sub-Saharan Africa during the past decade have been investigated by partial genomic sequencing across the VP1/2A region of the polioviral genome. Sequencing templates were generated by single-step reverse transcription PCR amplification of the viral RNA using poliovirusspecific primers. Seven poliovirus genotypes, circulating in different geographical regions during different periods, were identified. Considerable genetic variation was exhibited by strains within several of these genotypes, indicative of sustained endemic transmission within individual countries. Two genotypes appear to be circulating in Africa at present ; one major genotype, which has been in circulation since at least 1980, covers a wide geographical region and includes countries in western, central and southern Africa.
