(18)F-FDG PET is a unique imaging technique for assessing the metabolic activity of synovitis. The PET findings are correlated with MRI and US assessments of the pannus in RA, as well as with the classical serum parameter of inflammation, CRP, and the synovium-derived parameter, serum MMP-3. Further studies are warranted to establish the place of metabolic imaging of synovitis in RA.
RésuméPropos. -Chez trois patients atteints de maladie de Horton (MH), la tomographie par émission de positons (TEP) au 18 F-fluoro-2-déoxy-D-glucose ( 18 FDG) a montré un hypermétabolisme artériel au niveau de localisations extratemporales asymptomatiques. Méthodes. -Une TEP au 18 FDG a été réalisée chez 3 patients présentant un syndrome inflammatoire inexpliqué. L'examen a été pratiqué chez des sujets à jeun, 60 min après injection intraveineuse de 222 MBq de 18 FDG en moyenne. Résultats. -Dans les 3 observations, la TEP a objectivé un hypermétabolisme polyartériel asymptomatique, entreprenant les vaisseaux cervicothoraciques et abdominaux. Chez 2 patients, une artérite gigantocellulaire (AGC) est histologiquement confirmée. Dans le troisième cas, le diagnostic de MH est retenu, notamment sur le résultat de la TEP. Après corticothérapie, les 3 patients ont présenté une réponse clinique et biologique complète avec disparition des anomalies métaboliques à la TEP de contrôle.
In conclusion, implementing hybrid SPECT/CT imaging requires new ways of working. It is highly rewarding from a clinical perspective, but it also proves to be a daily challenge in terms of management.
