A woman with a history of systemic lupus erythematosus presented with extensive bilateral strokes due to acute inflammatory, occlusive large vessel disease affecting several aortic branches including the carotid, subclavian, renal, and iliac arteries. We quantitatively characterized the arterial inflammation in this patient and compared it with the inflammatory infiltrates from 22 patients with conventional atherosclerosis. Profound histomorphologic differences from conventional atherosclerosis (predominance of CD8-positive lymphocytes, relative absence of macrophages, no ectopic neovascularization, no signs of plaque hemorrhage, concentric instead of eccentrical stenosis) suggest that this patient's accelerated arteriopathy was precipitated by pathogenic events other than conventional atherosclerosis.
Es handelt sich um eine 62-jährige postmenopausale Patientin, welche mit Verdacht auf Pankreatitis (DD: Pankreaskarzinom) hospitalisiert war. Anamnestisch besteht Status nach Cholezystektomie, eine arterielle Hypertonie sowie chronische Rückenschmerzen. Klinisch bestanden gürtelförmige Oberbauchschmerzen sowie eine leichte epigastrische Druckdolenz. Die Patientin ist Zeugin Jehovas. Die Computertomographie zeigte nebenbefundlich eine tumoröse Raumforderung im kleinen Becken, welche sich als lobulierte Masse einzelner, schlecht abgrenzbarer Knoten darstellte und welche den Uterus allseitig umgab (Abb. 1). Das kleine Becken wurde von diesem Tumor deutlich ausgefüllt, wobei der Durchmesser der einzelnen Knoten bis zu 5 cm groû war. Hierdurch kam es zu einer Verdrängung der Dünndarmstrukturen nach kranial sowie zu einer Pelottierung mit leichter Linksverlagerung des Rektums. Erstaunlicherweise ergab sich aufgrund der negativen Dichtewerte von ± 10 bis ± 30 HE weder der Aspekt eines zystischen Tumors noch von typischem Weichteilgewebe. Vielmehr deutete der Befund auf das Vorhandensein von Fettgewebe. Der Uterus selber war normal groû abgrenzbar, auch erkannte man keinen Aszites und keine pathologischen Lymphknoten retroperitoneal. Bei der bimanuellen Palpation kann man deutlich den Uterus als normal groû, mobil und indolent abgrenzen. Die Adnexbereiche Zusammenfassung Wir berichten über eine 62-jährige Patientin mit der seltenen Diagnose eines Lipoleiomyoms des Uterus. Die Läsion wurde als Zufallsbefund entdeckt und konnte präoperativ nicht diagnostiziert werden. Es wird ausführlich über die bildgebenden Verfahren sowie die histologischen Gegebenheiten dieser sehr seltenen Tumorentität berichtet. Sowohl die möglichen Entstehungsmechanismen als auch die klinisch pathologischen Besonderheiten werden besprochen.
