Pendahuluan. Skor Sokal dan Hasford berkembang sebelum era terapi tirosine kinase inhibitor (TKI) untuk memberikan prediksi dan prognosis hasil terapi pasien leukemia granulositik kronis (LGK). Hingga saat ini, keunggulan skor Sokal dan Hasford masih diperdebatkan dalam memprediksi hasil terapi di era TKI. Imatinib mesylate merupakan TKI lini pertama dalam terapi LGK. Penelitian ini bertujuan untuk mengetahui skor prediktif yang lebih unggul dalam memprediksi respons hematologi maupun respons molekular pasien yang diterapi dengan imatinib mesylate. Metode. Penelitian ini merupakan penelitian survei, data yang diambil secara retrospektif dari rekam medis. Sampel penelitian adalah pasien LGK fase kronis yang sekurang-kurangnya telah mendapat terapi imatinib mesylate selama 12 bulan. Perhitungan skor Sokal maupun Hasford dilakukan berdasarkan data pasien pada saat mendapat imatinib pertama kali. Untuk melihat perbedaan distribusi antara kedua skor, digunakan uji statistik marginal homogenity. Untuk melihat proses perubahan respons pada pasien yang diobati dengan imatinib dilakukan uji kesesuaian menggunakan chi-square dan fischer dengan nilai p signifikan yaitu p<0,05. Hasil. Didapat sampel sebanyak 52 orang dengan rasio laki-laki : perempuan yaitu 1,4 : 1. Rerata usia saat terdiagnosis yaitu 43 tahun. Distribusi skor pada skor Sokal terbanyak pada kelompok risiko tinggi sedangkan skor Hasford pada kelompok risiko sedang. Prediksi skor Sokal terhadap pencapaian complete hematologic response (CHR) didapat masing-masing 44,5%, 38,1%, dan 9,1% untuk risiko rendah, sedang, dan tinggi secara berturut-turut. Prediksi skor Hasford terhadap pencapaian CHR didapat masing-masing 47,1%, 23,1%, dan 0% untuk risiko rendah, sedang, dan tinggi secara berturut-turut. Prediksi skor Sokal terhadap pencapaian major molecular response (MMR) didapat masing-masing 44,4%, 58,8%, dan 100% untuk risiko rendah, sedang, dan tinggi secara berturut-turut. Prediksi skor Hasford terhadap pencapaian didapat masing-masing 78,6%, 47,6%, dan 62,5% untuk risiko rendah, sedang, dan tinggi secara berturut-turut. Dari uji statistik Chi-Square tidak ada satupun baik skor Hasford maupun skor Sokal yang berbeda secara bermakna (berdasarkan tinggi rendahnya nilai skor dikaitkan dengan respons hematologi pada bulan ke-3 dan respons molekular pada bulan ke-12) dengan nilai p > 0,05. Simpulan. Tidak ada perbedaan antara skor Hasford dan skor Sokal dalam memprediksi respons hematologi maupun respons molekular pasien LGK kronis. Skor Sokal maupun skor Hasford dapat digunakan untuk memprediksi respons hematologi maupun respons molekular pada pasien LGK fase kronis yang diterapi dengan imatinib mesylate.
Chemotherapy-induced mucositis is a side effect of chemotherapy that often occurs in patients with solid tumors and lymphoma. Oral mucositis can affect nutritional status and the risk of infection, both local and systemic. Antioxidant Vitamin E is beneficial for the prevention and therapy of both oral and gastrointestinal mucositis. Aim: To determine the effect of vitamin E therapy on the incidence of oral mucositis in non-Hodgkin lymphoma (NHL) patients receiving chemotherapy. Methods: This is a single-blind experimental study in 62 NHL patients undergoing chemotherapy who meet the inclusion criteria. Patients who met the inclusion criteria were randomly divided into 2 groups, namely 31 patients (treatment group) received vitamin E 400 mg / IU per day for 7 days and 31 patients in the placebo group. The incidence and grade of oral mucositis were observed on day 7. Statistical analysis used Chi-Square and Mann Whitney test according to the data type. Results: a total of 67% NHL patients were male, most of whom were over 46 years of age and as many as 50% of patients used chemotherapy regimens RCHOP and CHOP. There was an incidence of oral mucositis in 35% in the placebo group and 12.9% in the treated group (p = 0.038). In the placebo group, there were 4.8% of patients with grade 2 and 3 oral mucositis, which was not found in the therapy group. Conclusion: treatment with vitamin E in NHL patients undergoing chemotherapy can prevent chemotherapy-induced oral mucositis and prevent its severity.
