A obesidade é uma temática recorrente nas reuniões e conferências de saúde, em virtude da ampla gama de alterações metabólicas e orgânicas proporcionadas por essa doença crônica não transmissível. As preocupações vão além do padrão estético e cultural, mas abrangem todas as repercussões cardiovasculares e endócrinas produzidos como resposta a inflamação crônica gerada pelo acúmulo excessivo de tecido adiposo. O tecido adiposo atua no organismo como um órgão endócrino através da liberação de adipocina, mediadores inflamatórios e citocinas, que atuam de forma ativa no processo inflamatório crônico, na disfunção endotelial e na resistência à insulina. A obesidade associada a resistência à insulina promove baixa de liberação de omentina, responsável por produzir vasodilação, auxiliar na regulação dos níveis séricos de insulina e promove inibição da resposta inflamatória vascular. Dessa forma, o organismo fica predisposto a intensa e constante lesão vascular, e ao desenvolvimento de diabetes mellitus tipo 2. As disfunções vasculares associadas ao excesso de lipoproteínas séricas predispõem a formação de placas de ateroma, responsáveis pela estenose e isquemia tecidual. Essas placas podem liberar fragmentos ou formar superfícies reativas formadoras de trombos, associados diretamente as doenças coronarianas, ao infarto agudo do miocárdio e acidentes vasculares encefálicos. Consequência a essa cascata de danos cardiovasculares os indivíduos ficam susceptíveis ao desenvolvimento de doenças incapacitante e altos índices de mortalidade. Sendo essa uma preocupação de saúde pública.
INTRODUÇÃOA obesidade é apresentada hoje como uma doença crônica não transmissível, de origem multifatorial e com repercussão sistêmica, ocasionada pelo desequilíbrio crônico entre o consumo alimentar e o gasto energético, além de diversos fatores ambientais e genéticos que influenciam nesse desequilíbrio. É caracterizada pela Organização Mundial da Saúde (OMS) como um acúmulo anormal ou excessivo de gordura corporal, sendo o parâmetro de classificação utilizado o índice de massa corporal (IMC) com valores iguais ou superiores a 30kg/m2 (BARROSO et al., 2017).Em conjunto com o IMC, a medida da circunferência abdominal é um critério que deve ser avaliado nos pacientes, visto que, a deposição de gordura visceral apresenta correlações com a síndrome metabólica e doenças cardiovasculares.Segundo as recomendações da International Diabetes Federation (IDF), os valores de circunferência abdominal acima de 90 cm para homens e 80 cm para mulheres já são considerados de risco (BALLIN et al., 2021) As doenças cardiovasculares se encontram entre as principais causas de óbito no Brasil e no mundo, superando as doenças infecciosas como causa de mortalidade e incapacidade em 2020. Tem sua causa multifatorial, e contribuindo para o declínio da qualidade de vida.A obesidade é determinada como o acúmulo de gordura corporal causado por um consumo de energia na alimentação, superior àquela usada pelo organismo para sua manutenção e realização das atividades do dia-a-dia, sendo assim é calculada pelo Índice de Massa Corporal (IMC) que é calculado dividindo-se o peso (em kg) pelo quadrado da altura (em metros), (IMC=peso (kg)/altura m x altura m). Menor que 18,5 -Abaixo do peso; Entre 18,5 e 24,9 -Peso normal; Entre 25 e 29,9 -Sobrepeso (acima do peso desejado); Igual ou acima de 30 -Obesidade (CEBRIM et al., 2009).Na obesidade, os depósitos de gordura corporal estão aumentados, apresentando uma elevação na expressão e secreção das adipocinas, proporcionalmente ao maior volume das células adiposas. Possuímos diferentes depósitos de gordura, entre eles: o tecidos adiposos visceral, subcutâneo abdominal, subcutâneo glúteo-femural e intramuscular, que possuem um grau metabólico e endócrino diferenciados, podendo interferir de forma específica nos processos inerentes à adiposidade corporal em obesos e diabéticos. O presente trabalho visa discutir sobre o papel endócrino e metabólico de cada compartimento de tecido adiposo, de modo a avaliar a contribuição dos mesmos nas complicações inerentes à obesidade.
