Objetivo: Comparar prevalência e grau de disfunção erétil (DE) entre os pacientes com Doença Renal Crônica (DRC) transplantados renais e em hemodiálise. Métodos: Estudo analítico e transversal, incluindo 92 indivíduos do sexo masculino em serviço de referência, com idade de 20 a 68 anos. Investigou-se presença de DE pelo Índice Internacional de Função Erétil (IIEF-5), e informações sobre idade, etiologia, medicações, tabagismo e tempo em substituição renal pelo prontuário. Resultados: A idade média foi 53.4±14.82 no grupo hemodiálise (HD) e 48.2±14.34 no grupo transplante (TR). Em HD, 78% apresentaram DE, e em 57.2% em TR. A classificação no IIEF-5 demonstrou 28% HD com DE moderada e 21.5% TR com DE leve/moderada. O tempo médio em hemodiálise foi 3.4 anos para HD e 4 anos para TR. O tempo em hemodiálise ≥ 4 anos afetou a pontuação IIEF-5 em ambos os grupos (p=0.0474). Faixa etária ≥ 50 anos teve associação com DE em HD (p=0.0411) e em TR (p=0.0045). Uso de inibidores de 5-fosfodiesterase foi encontrada em 30.9% TR e 10% HD, tendo impacto sobre o IIEF médio de ambos os grupos (p=0.0193). Conclusão: A prevalência de DE é alta em pacientes em terapia de substituição renal, sendo mais frequente na hemodiálise. O transplante renal aparece como alternativa para melhora e estabilização da função renal nos pacientes estudados, com impacto positivo na saúde mental e bem-estar dos renais crônicos.
