Girls were more affected by chorea; heart valves and joints were equally affected and represented the major clinical features; no specific pattern of joint involvement (eg.: migratory arthritis) could be labeled as typical; and strict adherence to Jones criteria, with compulsory documentation of a previous GASI, may lead to underdiagnosis of ARF.
A case of juvenile cutaneous polyarteritis nodosa (cutaneous PAN) is presented. Since early infancy the child underwent attacks of fever and cutaneous rash that occasionally progressed to gangrene and amputations of distal portions of toes and fingers. Although occasional episodes of high blood pressure and persistence of moderate eosinophilia were present, the clinical pattern was mostly restricted to the musculoskeletal system and skin. The authors discuss the definition of the disease and its present therapeutic possibilities, calling attention to a feature not referred in previous reports: the unique co-existence of cutaneous PAN plus antiphospholipid antibodies (aPL) and perinuclear antineutrophil cytoplasmic antibodies (p-ANCA).
Objetivo: Relatar o caso de uma criança portadora de doença de Bruton que apresentava a incomum associação com artrite asséptica crônica, clinicamente indistinta da artrite reumatóide juvenil. Métodos: O paciente, um lactente de um ano e quatro meses de idade, foi atendido no ambulatório de Reumatologia Pediátrica do Departamento de Pediatria do HC-UFG e no Hospital da Criança, (Goiânia-GO). Foram avaliadas as características clínicas e laboratoriais, inclusive a evolução do caso e a resposta à terapêutica. Esses dados foram então comparados com relatos anteriormente publicados na literatura mundial, analisados a partir de levantamento utilizando o sistema MEDLINE como base de dados. Resultados: O paciente apresentava, desde os seis meses de idade, quadros infecciosos de repetição, que respondiam mal à antibioticoterapia. Há 40 dias da internação, havia se instalado artrite em joelho esquerdo. O diagnóstico de Bruton foi ratificado com base na dosagem sérica de imunoglobulinas, resposta à reposição endovenosa com gamaglobulina e outros exames laboratoriais indiretos. Conclusão: Apresentamos um caso da incomum associação de doença de Bruton e monoartrite crônica asséptica, semelhante à artrite reumatóide juvenil. O reconhecimento precoce do caráter asséptico incomum de envolvimento articular possibilita um tratamento eficaz, baseado na reposição adequada de imunoglobulinas, e evita internações e antibioticoterapia inadequadas.
