Four cases of acute interstitial pneumonia (AIP) are described with special emphasis on clinical background, lung imaging and bronchoalveolar lavage findings.A retrospective chart review of four patients with histologically-proven AIP, diagnosed between 1998 and 2000, was carried out. Clinical data, bronchoalveolar lavage (BAL) findings, high-resolution computed tomography (HRCT) and histological features were analysed.Three patients died and only one is in follow-up. HRCT showed areas of ground glass attenuation and alveolar consolidation in all patients. Histology, documented by open lung biopsy or autopsy specimens, was consistent with the organising form of diffuse alveolar damage pattern.BAL findings were characteristic, with a huge neutrophilia associated with scattered atypical type II pneumocytes collected in clusters with extracellular amorphous material (fragments of hyaline membranes) observed in two out of three cases.In this paper, four cases of acute interstitial pneumonia are reported in detail. The poor prognosis associated with this entity has been confirmed and the possible diagnostic role of the bronchoalveolar lavage is emphasised.
Arq Bras Endocrinol Metab vol 45 nº 2 Abril 2001 123 RESUMOAndropausa é uma designação inapropriada para o quadro clínico resultante do declínio progressivo da produção androgênica encontrado em pelo menos 20% dos homens com idade entre 60 e 70 anos, e que algumas vezes se inicia a partir dos 50 anos. Uma designação mais adequada é insuficiência androgênica parcial do homem idoso. Como a produção de testosterona diminui regularmente, em homens de 75 anos os níveis médios de testosterona são somente 65% daqueles dos adultos jovens, sendo que pelo menos 25% destes idosos apresentam níveis subnormais de testosterona biodisponível. A etiologia deste declínio da testosterona dependente da idade é multifatorial e envolve alterações testiculares primárias, disfunção da regulação neuroendó-crina das gonadotropinas, elevação das concentrações séricas de globulina ligadora de hormônios sexuais e redução da sensibilidade dos receptores androgênicos. A senescência é acompanhada de uma série de sinais e sintomas, muitos deles bastante semelhantes aos observados em hipogonádicos jovens. Este quadro clínico complexo pode se dever aos efeitos conjuntos do próprio processo de senescência e de doenças intercorrentes. No entanto, existem evidências que o declínio dos níveis de testosterona próprio da idade é, pelo menos em parte, codeterminante deste quadro clínico, visto que a reposição androgênica tem mostrado efeitos favoráveis em mais de 30% destes idosos sintomáticos. No momento, esta reposição hormonal deveria somente ser considerada em presença de níveis séricos de testosterona abaixo dos limites normais mínimos para adultos jovens, acompanhada de sinais inequívocos de insuficiência androgênica, na ausência de outras causas reversíveis de hipoandrogenismo e após a exclusão de contra-indicações. ABSTRACTAndropause is a misnomer for the clinical picture resulting from the progressive decrease in androgen production observed in at least 20% of 60-70 year-old males, sometimes starting at the age of 50. A more appropriate designation is partial androgen deficiency of the aging male. As testosterone production declines steadily, at age 75y. plasma testosterone levels are only 65% of levels in young adults, whereas over 25% of these men have subnormal bioavailable testosterone levels. The ethiology of this age-related testosterone decline is multifactorial and involves primary testicular changes, disturbed neuroendocrine gonadotropin regulation, increased sex-hormone binding globulin serum concentrations and reduced androgen receptor sensitivity. Aging is accompanied by a series of signs and symptoms, many of which are rather similar to those observed in young hypogonadal males. This complex clinical picture may arise from the concurrent effects of the aging process per se and intercurrent diseases. There is a group of middle-aged men with sexual dysfunction that presents reduced serum testosterone levels and normal to low levels of prolactin and gonadotropins, but without any dermonstrable hypothalamicpituitary anatomic abnormality. This condition is considered a form of idiopathic hypogonadotropic hypogonadism, probably of hypothalamic origin. We present and discuss two patients with similar features in whom sexual dysfunction preceded actual hypoandrogenism. We conclude that in these two patients hypoandrogenism was the consequence of a psychoneuroendocrine disturbance due to the sexual dysfunction. 1,2). No entanto, têm sido descritos casos de pacientes não idosos com DS e hipoandrogenismo secundário na ausên-cia de lesão orgânica hipotálamo-hipofisária, nos quais a origem destes achados permanece obscura. Estamos apresentando os casos de dois destes pacientes, e discutindo as possíveis causas do hipoandrogenismo e sua relação com a DS. Caso lPaciente de 46 anos, casado e com um filho de 5 anos, visto em abril de 1997 com libido normal e queixas de disfunção erétil e ejaculação precoce. Sendo professor universitário em fase de doutoramento, encontrava-se sob stress intenso (sic), que era agravado pela própria DS. Não fumava, mas referia ingestão diária de bebidas alcoólicas. Seu IMC era 21 kg/m 2 . Os exames clínico e da genitália resultaram normais. A capacidade erétil era normal, comprovada por um teste de injeção intracavernosa de prostaglandina.
The aim of this study was to analyse the reliability of the GH -clonidine test (GH-CT) in the diagnosis of a relative GH insufficiency in an infertile male population. Twenty seven pacients were classified in 3 groups according to clonidine dosage. Groups I (clonidine 0.150mg) and II (clonidine 0.200mg) had 9 and 6 oligozoospermic patients, respectively. Group III (clonidine 0.400mg) had 5 oligozoospermic (subgroup IIIa) and 7 normozoospermic patients (subgroup IIIb). Thirty one GH-CT were performed and 29 of them were included in the study. Five (17.2%) normal GH-CT were observed (1 in group I, 1 in group II and 3 in subgroup IIIb). GH responses at 60, 90 and 120 min after clonidine stimulation showed no significant differences in groups I, II and III, neither between subgroups IIIa and IIIb. We conclude that the GH-CT is of no use in the diagnosis of a relative GH insufficiency in the infertile male, irrespective of the clonidine dosage. The reason is the clonidine inability to carry out a reliable GH stimulation in the adult male.
