Aim: The aim of the paper was to create a literature review of valid and reliable delirium screening instruments administered by nurses for hospitalized patients. Design: Literature review. Methods: An advanced search of three licensed electronic databases (EBSCO, MEDLINE, PROQUEST) was selected. Twenty-one research studies complied with the inclusion criteria. Results: The predictive validity of ten delirium screening measuring tools were studied. The best predictive validity was found in the Nursing Delirium Screening Scale (Nu-DESC), with sensitivity values ranging from 32% to 100%, and specificity from 83% to 100%, with highest value for reliability of 0.94. The Delirium Observation Screening Scale (DOS) came second, with sensitivity from 25% to 97%, and specificity from 89% to 98.4%, with highest value for reliability of 0.77. The 4AT tool had sensitivity values from 83.3% to 90%, and specificity from 84% to 86.3%, with highest value for reliability of 0.99. Conclusion: The best screening tool for evaluating delirium by nurses was the Nu-DESC, followed by the DOS, and the 4AT. We recommend testing the predictive validity and reliability of selected screening tools administered by nurses in conditions of Czech clinical practice.
Tlakové léze představují významné riziko a následně komplikaci pro pacienty, kterým je v rámci léčby přiložena fixace (sádra, sádrové dlahy, krční límce, ortézy). Cíl: Vytvořit přehled odborných informací popisujících tlakové léze vzniklých v důsledku přiložení fixace (sádra, sádrové dlahy, krční límce, ortézy). Metody: Pro tvorbu přehledu bylo zvoleno jednoduché vyhledávání ve dvou licencovaných a elektronických databázích a jednom nakladatelství (EBSCO, PROQUEST, SOLEN). Vyhledávání probíhalo v měsíci lednu a únoru 2019. Zařazovací kritéria splňovalo sedmnáct odborných publikací. Výsledky: Tlakové léze jsou způsobeny zejména vysokým působením tlaku, zevními a vnitřními faktory. Nesprávné a nedostatečné umístění fixace, která intenzivně tlačí na kůži, má za následek nedostatečný arteriální průtok krve. Dochází ke tkáňové perfuzi a následně vzniku tlakové léze. V horším případě může dojít k tvorbě trombu. Závěry: Monitorování kůže patří mezi základní činnosti ošetřovatelského personálu během přijetí a následné péče o pacienta v nemocnici. Vzdělávání ošetřovatelského personálu o správném přikládání kompenzačních pomůcek a péče o kůži, by měla být prioritou. Objective: Determine the results for inter-rater reliability for items in two delirium screening measuring instruments (Delirium Observation Scale, Nursing Delirium Screening Scale). Design: Prospective comparative study of measuring instruments. Methods: Data were collected between August 2018 and August 2019 at the Trauma Clinic, Teaching Hospital, Olomouc, Czech Republic. Study sample included 50 patients with locomotive apparatus trauma, assessed by two raters -general nurses. To assess the inter-rater reliability the study used Cohen's kappa (κ) method. Results: For the Delirium Observation Scale, the agreement between the two raters for two of the items was 98% (κ = 0.790); for all other items the agreement was 100% (κ = 1.0). The percentage rate of agreement between both raters using the Nursing Delirium Screening Scale was 100% (κ = 1.0) for all five items. Conclusions: For both screening measuring instruments a high level of reliability has been established. Nevertheless, the highest agreement between the raters has been achieved for the Nursing Delirium Screening Scale. The recommendation is to further test the screening delirium measuring instruments in a Czech clinical setting.
Individuální rozdíly v cirkadiánních typech mají vliv na diurnální preference (DP) sestry a její schopnost adaptace na práci na směny. Sestry večerní DP se lépe přizpůsobují nočním směnám oproti sestrám ranní DP. V souvislosti se směnným režimem sestry sdělují problémy se spánkem, rodinné a společenské problémy. Cíl: Zjistit, zda měla vliv DP sester z různě zaměřených pracovišť nemocnic na jejich směnnost a spánek. Dílčími cíli bylo zjistit zastoupení sester v DP a zjistit, jaký mají vztah ke směnnosti a spánku sester. Zjistit reliabilitu použitého dotazníku. Metodika: Kvantitativní průřezová studie byla založena na anonymním dotazníkovém šetření. Soubor 726 sester byl rozdělen na 4 skupiny podle pracoviště. Dotazník Kompozitní škála ranních a večerních typů byl využit pro stanovení DP sester, byl doplněn otázkami vztahujícími se ke směnnému režimu, spánku a anamnestickými daty. Pro zpracování dat byla použita parametrická a neparametrická statistika. Výsledky: V souboru bylo zastoupeno 23 % sester ranní DP, 5 % večerní DP a 72 % neutrální DP. V ranních směnách se velmi dobře cítily sestry ranní DP (p < 0,001) a v nočních směnách sestry večerní DP (p < 0,001). Po spánku po noční směně se cítily hůře sestry ranní DP. Vyšší hodnota v počtu hodin spánku po noční směně byla zjištěna u sester večerní DP (p < 0,001). Reliabilita škály byla uspokojivá (0,84). Závěr: Na spánek u sester ve směnném provozu má vliv typ DP. Problémy se spánkem nelze podceňovat a je třeba prevence, např. vhodné ergonomické plánování pracovních směn.
