Actualmente se vive una crisis sanitaria de alcance global, catalogada por la Organización Mundial de la Salud (OMS) como pandemia (1). Se reportan al 26 de julio del 2020: 646 996 víctimas mortales y sobrepasa los dieciséis millones de casos confirmados en el mundo (2) En Sudamérica el Perú, después de Brasil, ocupa el segundo lugar con 379,884 casos y 18,030 defunciones (2). Por las características propias de esta pandemia, el personal de salud se enfrenta a estresores laborales intensos, tales como largas jornadas de trabajo, sobrecarga laboral, instrucciones y medidas de seguridad estrictas, necesidad permanente de concentración y vigilancia, escasez de equipamientos de protección entre otros (3,4). Se incluye además el colapso de un sistema sanitario precario, una mayor duración de la cuarentena, frustración, aburrimiento, pérdidas financieras (5) información insuficiente y poco claras para poder tomar acciones adecuadas (6,7) percepción de falta de transparencia de los funcionarios públicos y del gobierno sobre la gestión de los recursos públicos (7). Esta situación, pone en riesgo la salud física y mental del personal sanitario, dando lugar a contagios por inadecuadas condiciones de bioseguridad e infraestructura sanitaria, complicaciones a su propia salud y muerte. La afectación psicológica se manifiesta con síntomas de ansiedad, depresión o trastornos por estrés postraumático (8) o traumatización vicaria derivada de la compasión hacia los pacientes que están atendiendo (9).
Objetivo: Determinar el nivel de satisfacción del usuario externo en un establecimiento de atención primaria de salud, durante la pandemia de COVID-19. Métodos: Estudio de tipo observacional, prospectivo y transversal, realizado en un establecimiento de primer nivel de atención del sistema sanitario peruano, durante julio a agosto del 2020. La muestra estuvo conformada por 120 usuarios externos. Se recolectaron datos generales y se administró una escala SERVQUAL del Ministerio de Salud, modificada al modelo SERVQHOS. Se aplicó análisis estadístico de nivel descriptivo. Resultados: De los participantes, fueron de sexo femenino (80,7%) y grupo de edad 41 a 50 años (41,0%). En general el 38,3% se encuentran medio satisfecho y 61,7% insatisfecho con los servicios de salud. En las dimensiones fiabilidad 69,2%; capacidad de respuesta 76,7%; seguridad 58,3%; empatía 60,8% y aspectos tangibles 87,5% de insatisfacción respectivamente. El 75,8% de los usuarios externos se encuentran insatisfechos con el trato que reciben en el servicio de salud. Conclusiones: Durante la pandemia de COVID-19, en promedio los usuarios externos se encuentran insatisfechos con la atención recibida, en un establecimiento de atención primaria de salud.
Objetivo: Conocer los niveles de salud autopercibida en trabajadores de enfermería del primer nivel de atención, durante la pandemia de COVID-19. Materiales y métodos: Estudio de tipo descriptivo y transversal, realizado en 32 establecimientos de primer nivel de atención de la Red de salud Ica, durante junio y julio del 2020. La muestra estuvo representada por 166 participantes entre enfermeros y técnicos de enfermería. Se recolectaron datos sociodemográficos y se administró el Cuestionario de Salud General de Goldberg de 12 preguntas. Se describieron las variables del estudio y se aplicó Chi-cuadrado para evaluar diferencias. Resultados: De los participantes, fueron de sexo femenino (80,7%); función del cargo enfermero (56,0%); condición laboral nombrado (67,5%) y grupo de edad 41 a 50 años (41,0%). La prevalencia autopercibida de mala salud, fue del 50%; las diferencias resultaron significativas según el sexo y función del cargo; no obstante en la condición laboral y los grupos de edad, las diferencias no fueron significativas. Los resultados varían según disforia general (ansiedad y depresión) y disfunción social o mal funcionamiento social. Conclusiones: Durante la pandemia de COVID-19, el nivel de mala salud autopercibida es de tendencia alta, existen diferencias estadísticas en algunas variables sociodemográficas. Es necesario promover acciones conjuntas que contribuyan al bienestar del trabajador de enfermería.
La Inteligencia Consciencial, término acuñado recientemente por Becerra (1), para referirse a la novena inteligencia múltiple de Gardner (2), recobra importancia en los tiempos actuales de crisis, por su relación positiva directa con la salud, la felicidad, el desempeño y la humanización de las instituciones sanitarias y sociedad en general. La Inteligencia Consciencial (IC) o Inteligencia Espiritual (IE), un constructo científicamente estudiado hace más de 20 años; (3) nos ayuda a encontrar el sentido de la existencia humana y el significado de la vida; de ahí, que la mayoría de autores proponen el uso de este conocimiento para resolver problemas cotidianos, buscando una vida mejor (4); por otro lado, autores como Amram, (5) sostienen que la IE, promueve el desarrollo de potencial humano y es necesario volverlo operativo. En ese contexto la IC, es definida por Becerra, (6) como “El conjunto de capacidades mentales que se ocupan de cuestiones existenciales, trascendentes y/o consciénciales de la condición de ser-humano y su conexión con el universo; para desarrollar facultades que conducen a la sensibilización, humanización y autorrealización del ser humano auténticamente genuino” la IC, le permite al hombre reflexionar sobre sí mismo, ver el mundo material desde otras perspectivas, facilita el autoconocimiento y permite crear consciencia.
Con el objetivo de conocer el nivel de satisfacción de los usuarios de consultorios externos de cuatro hospitales públicos de la región Ica luego de la implementación del plan «Cero colas» se encuestó a 1067 usuarios, utilizando un instrumento que indagaba sobre las características generales, calificación del servicio y satisfacción global. Se encontró un nivel de satisfacción global «muy satisfecho/satisfecho» del 72,5% en el Hospital de Apoyo de Nazca, 70,6% en el Hospital Santa María del Socorro de Ica, 72,1% en el Hospital Regional de Ica y 73,7% en el Hospital San Juan de Dios de Pisco. Se concluye que la implementación del plan «Cero Colas» mejoró el nivel de satisfacción global, el buen trato, el cumplimiento del horario de atención médica y la claridad e información brindados por encima del estándar esperado.
