Introduction: Asthma is characterized by a narrowing and inflammation of the bronchi, with symptoms of dyspnea, fatigue and exercise limitation. Physical therapy includes inspiratory muscle training and breathing exercises, given that an increase in inspiratory muscle strength and resistance can improve the symptoms of the disease. Objective: To describe the effects of inspiratory muscle training (IMT) and breathing exercises in children with asthma. Methods: This is a systematic review of the literature using the Cochrane, PubMed Scopus e Web of Science databases. The following descriptors were used: asthma, inspiratory muscle training, breathing exercises and child in Portuguese, English and Spanish. Two independent evaluators screened studies that used breathing exercises and IMT in children with asthma. Results: Of a total of 312 titles, eight studies were included, of which six are randomized clinical trials and two are observational studies All the studies included breathing exercises, with the objective of adjusting breathing patterns and pulmonary ventilation, reducing pulmonary hyperinflation, bronchospasm and sensation of dyspnea. However, as these exercises were not performed solely, the effects of this intervention could not be verified. Two studies performed IMT and showed an increase in maximal respiratory pressure. Conclusion: Breathing exercises are widely used in clinical practice as part of the management of asthma in children; however it is not possible to measure the effects in this population. IMT seems to improve inspiratory and expiratory muscle strength, but its indication in the pediatric population is not a standard procedure.
Objective To compare performance and cardiorespiratory responses in field tests performed by healthy children, with and without the use of a portable gas analyzer (PGA), and to analyze the reproducibility of the tests. Method The study included healthy children aged 6 to 14 years old. The sample was divided into children who underwent field tests (modified shuttle walk test [MSWT], 6‐minute walk test [6MWT], and ADL‐Glittre for pediatrics [TGlittre‐P]) without PGA (GS) and children who carried the equipment K4b2—COSMED, Rome, Italy (GK). Results A total of 120 children participated in the study, 31 in the GK (mean age 10.16 ± 1.81 years old) and 89 in GS (mean age 10.1 ± 1.79 years old). The groups did not differ from each other and there was no significant difference in the performance of the 6MWT and MSWT intergroup (P > .05), but the significant alteration in the variables of the cardiorespiratory response was identified. In the TGlittre‐P, the GK completed the test in a shorter time than the GS (2.69 ± 0.38 minutes vs 2.91 ± 0.40 minutes) and both presented alterations in most cardiorespiratory variables. The intragroup reproducibility showed concordance in all tests, and there was no concordance between groups (P > .05). Conclusion The use of PGA had a lower time spent on TGlittre‐P. In the 6MWT and MSWT, there was no difference in cardiorespiratory performance and response with and without the use of PGA. The three field tests were reproducible, but the same test, with and without the use of PGA, did not show agreement.
Introdução: Objetivo: analisar a relação entre força muscular respiratória(FMR) e capacidade de atingir valores preditos na manobra espirométrica de capacidade vital lenta(CVL)em crianças em idade escolar. Método: pesquisa observacional transversal, incluiu crianças saudáveis entre 6 e 12 anos, cuja higidez controlou-se por meio de espirometria normal(Pneumatógrafo Jaeger-Master Scope IOS®), segundo American Thoracic Society (2005) e questionários de saúde (ISAAC e histórico de saúde). As crianças foram divididas em GCVL: aquelas que atingiram 80% na manobra de CVL;e como GCON: as que não atingiram este valor. As pressões máximas inspiratória (PI max ) e expiratória (PE max ) foram obtidas por meio de manovacuometria (MVD 300 Globalmed®). Após verificação da normalidade dos dados, aplicouse o teste de Spearman para análise da relação entre FMR com parâmetros de CVL. Os dados de PI max e PE max foram comparados entre os grupos, com o teste de Mann-Whitney. Adotou-se nível de significância de 5% (p<0,05). Resultados: participaram 83 crianças com média de idade de 8,54 ±1,95anos, sendo 44 no GCVL (25 meninas). Este grupo apresentou maiores valores absolutos e em percentual do predito (%)de capacidade inspiratória (CI), volume de reserva expiratório (VRE) e FMR. Houve relação entre % de CI e VRE com PE max . No GCON, (39 crianças /20 meninas) houve relação entre valores absolutos de CVL, CI, VRE e PI MAX e destes dois primeiros parâmetros e da % de CI com a PE MAX .Conclusões: houve relação entre a FMR e a capacidade de atingir 80% do predito na CVL, sendo que essa porcentagem foi alcançada pelas crianças com valores superiores de FMR.
Introdução: O teste de caminhada de seis minutos (TC 6 ) é comumente utilizado na faixa etária pediátrica, e variáveis preditivas e valores de referência têm sido estudados para o teste.Objetivo: Apresentar os achados na literatura sobre a determinação de variáveis preditivas e valores de referência para o TC 6 na população pediátrica.Materiais e Métodos: Revisão de literatura através de pesquisa nas bases de dados eletrônicas PubMed e EBSCO utilizando as palavras-chave: “child”, “six minute walk test”, “reference values” e “6-minute walk test”, buscando estudos que apresentassem valores e/ou equações de referência e apresentação de variáveis preditivas. Não foi utilizada restrição temporal e de idioma nas publicações.Resultados: De um total de 20 estudos, foram selecionados 11, os quais incluíram indivíduos entre 4 e 18 anos. A idade foi a variável de mais frequente relação com o desempenho no teste, seguida de massa corporal, estatura, sexo, variação da frequência cardíaca, índice de massa corporal e nível de atividade física . A altura foi a única variável preditiva comum em todos os estudos. Nota-se que diferentes metodologias e modificações são empregadas na realização do TC 6 .Conclusão: Diferentes equações de referência, bem como variáveis preditivas, são identificadas, sendo apenas a estatura comum nas diferentes publicações analisadas. Mesmo o TC 6 sendo um teste comumente utilizado, não há metodologia de aplicação e padronização em pediatria.
