A patient-centered educational video explaining aneuploidy testing options improved knowledge and self-reported understanding of the information in women younger than 35 years with no change in patient satisfaction. No difference was seen for women 35 years or older, likely as a result of genetic counseling provided to these women.

The occurrence of LGA remains very high in contemporary U.S. women with Type 1 DM, despite the use of CGM and overall good glycemic control. Neither HbA1c nor glycemic variability predicted fetal overgrowth or birthweight. Since LGA is a key driver of maternal and newborn complications in pregnancies with Type 1 DM, our data emphasize the importance of investigating both glucose-dependent and glucose-independent underlying mechanisms.

Transient myeloproliferative disorder (TMD), also known as transient acute myelopoiesis, is a myeloproliferative condition that occurs in the perinatal period in up to 10% of patients who have Down syndrome. Because of the perinatal presentation of this disorder, neonatal clinicians should be familiar with its clinical presentation, management, and outcomes. Affected patients develop severe myeloproliferation of megakaryocytic precursor cells, leading initially to hepatosplenomegaly and liver dysfunction. If the disorder develops prenatally, affected fetuses may also develop hydrops. The disorder requires the presence of 2 genetic abnormalities: trisomy of the gene ERG, located on chromosome 21, and a mutation of the GATA1 gene. The disease typically regresses spontaneously if the patient receives supportive care through the acute illness, although overall mortality of TMD is about 20%.

Urinary symptom scores as measured by the UDI-6 did not differ between women randomized to bladder flap or omission of the bladder flap, but the proportion of women with urinary symptom bother was significantly higher among those who received a bladder flap.

Objective: To compare the incidence of Group B Streptococcus (GBS) conversion from a negative antepartum to a positive intrapartum culture among women who self-identify as non-Hispanic black, Hispanic, or non-Hispanic white. Study Design: This was a prospective cohort study of women with a negative rectovaginal GBS culture within 35 days of enrollment. An intrapartum rectovaginal swab was collected and cultured for GBS. Data were compared with chi-square, Fisher’s exact, or Wilcoxon rank-sum test. Modified Poisson regression was used. Results: We enrolled 737 women; 75.4% were non-Hispanic white, 17.6% were non-Hispanic black, and 6.9% were Hispanic. Non-Hispanic black women were more likely to convert to GBS positive than non-Hispanic white women, 9.2% as compared to 5.3% (RR: 2.0; 95% CI: 1.02–3.8). Conclusion: The increased incidence of positive intrapartum GBS cultures among non-Hispanic black women suggests that non-Hispanic black race is a risk factor for GBS conversion in the late third trimester.

Babesiosis during pregnancy is an uncommon cause of hemolysis during pregnancy and may have a similar presentation to hemolysis, elevated liver enzymes, and low platelets (HELLP) syndrome. We report a case of a 34-year-old healthy gravida 3 para 1-0-1-1 at 38 weeks of gestation who was transferred to our tertiary care center for induction of labor for suspected HELLP syndrome. Peripheral blood smear from the referring hospital showed intraerythrocytic ring forms consistent with Babesia microti infection. She was treated with clindamycin and quinine and had a vaginal delivery of a healthy infant. This case demonstrates that although uncommon, babesiosis should be on the differential for imitators of HELLP syndrome to optimize treatment and outcomes.

Carney complex is a rare cause of hypercortisolism and hypertension during pregnancy. It should be considered when features of Cushing syndrome and severe hypertension are present.
