Objective:to investigate the influence of three different rotary systems for cervical flaring on establishment of the real working length. Material and methods: Thirty mandibular first molars were submitted to conventional endodontic access and initial working length measurement, followed by irrigation/suction of the pulp chamber with 5% sodium hypochlorite. Teeth were randomly divided into 3 groups (n=10) and cervical flaring of the mesiobuccal canals were performed using one of the following instrument systems: Group 1 – Gates-Glidden burs; Group 2 – Orifice Openers; Group 3 – La Axxess system. Two subsequent numbers of instruments of each rotary system were used and the final working length was recorded. A digital calyper was used to record the working length, in millimeters, to investigate a possible discrepancy between initial and final measurements.Results:Analysis of variance (Anova) na Tukey test revealed statistical difference between Group 1 and Groups 2 and 3 (p£ 0.05).Conclusions:all groups presented shorter working length after cervical flaring; groups prepared with instruments La Axxes and Orifice Opener presented the best results among the systems studied.
Aim: This study evaluated the influence of surgical undersized drilling on insertion torque of an implant system at low bone density. Methods: Implant site preparations were made in two polyurethane foam blocks with different densities, where two preparation techniques were considered: Control group (C): conventional drilling, following the manufacturer's instructions; and Group (E): undersized drilling, experimental technique using a final surgical drill with reduced diameter. The artificial bone blocks were selected based on density: D 1 (0.64 g/cm 3 ) and D 2 (0.32 g/cm 3 ). Three groups were considered according to the preparation technique and bone density used (n=10): CD 1 -conventional drilling technique in artificial bone with higher density; CD 2 -conventional drilling technique in artificial bone with lower density; and ED 2 -undersized drilling in artificial bone with lower density. Externalhexagon implants (11 × 4 mm) were inserted and the insertion torque values were measured using a digital torque-meter. Data were submitted to one-way ANOVA followed by Tukey's test (α=0.05). Results: The results showed significant statistical differences between groups (p<0.001), where the highest insertion torque was found for the Group CD 1 (48.9 Ncm) followed by Group ED 2 (22.6 Ncm) and Group CD 2 (11.7 Ncm). Conclusions: The undersized drilling technique for implant site preparation leads to increased insertion torque on low bone density.
R e s u m oObjetivo: O objetivo deste trabalho foi verificar o ganho ósseo intrasinusal obtido com as técnicas de osteotomia, taxas de sucesso de implantes e taxas de perfuração de membrana sinusal.Metodologia: Foi realizada uma busca retrospectiva na literatura sobre o tema em bases de dados eletrônicas e excluídos os artigos que não apresentavam relevância científica. Conclusão:A técnica da osteotomia se mostrou relevante cientificamente em todos os itens pesquisados. A maxila posterior edêntula, usualmente, apresenta limitações anatômicas para a instalação de implantes osseointegrados. Os principais obstáculos para a realização desses procedimentos na maxila são a reabsorção do osso alveolar, a pneumatização do seio maxilar em todas as suas direções e ainda uma diminuição na densidade óssea, classificado como tipo III ou IV (1,2). Tornando o osso medular menos denso, esse processo pode ser acelerado pelo uso de próteses removíveis, agravamento de doença periodontal ou ainda exodontias traumatizantes, acarretando assim a necessidade de cirurgias de enxerto para a reconstrução óssea e posterior instalação dos implantes. Contudo, essas cirurgias são um tanto quanto invasivas, traumáticas e demoradas, o que limita muito a sua aplicabilidade rotineira.A elevação da membrana de Schneider para aumento do seio maxilar foi primeiramente descrita na década de 80, como um caminho possível para restaurar essa região problemática (2,3). Outro autor desenvolveu uma técnica cirúrgica usando osteótomos, indicada para osso residual de 5 a 6 milímetros (mm) abaixo do assoalho sinusal e de baixa densidade (4-6). A técnica de levantamento sinusal atraumático descrita por Summers foi desenvolvida com o intuito de realizar a cirurgia sem o rompimento da membrana de Schneider, permitindo assim, a colocação de implantes mais longos, além de uma possível indução na melhora da densidade óssea, promovendo uma compactação da parte medular do tecido ósseo, beneficiando o prognóstico destes implantes e da reabilitação protética, diminuindo o tempo clínico e o risco cirúrgico, o dano tecidual, sem comprometimento da irrigação sanguínea local com menor invasividade reduzindo o custo cirúrgico e período de cicatrização para aplicação da carga protética (4-9).O objetivo deste artigo foi verificar o ganho ósseo intrasinusal com diferentes técnicas de osteotomia, verificar as taxas de sucesso dos implantes colocados com a técnica e as perfurações de membrana sinusal relativas ao procedimento.
