The time from cardiac catheterization to cardiac surgery is not a risk factor for the development of postoperative acute kidney injury even in patients with other risk factors. Surgical intervention should not be delayed in emergency or urgent cases. The optimization of renal function seems to be the correct strategy in clinically stable patients with risk factors for acute kidney injury.
Background Correlation of increased copeptin levels with various cardiovascular diseases has been described. The clinical use of copeptin levels in patients with hypertrophic cardiomyopathy (HCM) has not been investigated before. Hypothesis In this study, we aimed to investigate the prognostic value of copeptin levels in patients with hypertrophic cardiomyopathy (HCM). Methods HCM was defined as presence of left ventricular wall thickness ≥15 mm in a subject without any concomitant disease that may cause left ventricular hypertrophy. Levels of copeptin and plasma N‐terminal probrain natriuretic peptide (NT‐proBNP) were evaluated prospectively in 24 obstructive HCM patients, 36 nonobstructive HCM patients, and 36 age‐ and sex‐matched control subjects. Blood samples were collected in the morning between 7 and 9 am after overnight fasting. Patients were followed for 24 months. Hospitalization with diagnosis of heart failure/arrhythmia, implantable cardioverter‐defibrillator implantation, and cardiac mortality were accepted as adverse cardiac events. Results Copeptin and NT‐proBNP levels were higher in the HCM group compared with controls (14.1 vs 8.4 pmol/L, P < 0.01; and 383 vs 44 pg/mL, P < 0.01, respectively). Copeptin and NT‐proBNP levels were higher in the obstructive HCM subgroup compared with the nonobstructive HCM subgroup (18.3 vs 13.1 pmol/L, P < 0.01; and 717 vs 223 pg/mL, P < 0.01, respectively). In multivariable logistic regression analysis, copeptin and NT‐proBNP levels remained as independent predictors of heart failure (P < 0.01 for both) and adverse cardiac events (P < 0.01 for both). Conclusions Copeptin and NT‐proBNP levels were significantly higher in patients with obstructive HCM, and higher levels were associated with worse outcome.

Renal cell carcinoma (RCC) is associated with a biological propensity for vascular invasion with extension of the tumor to the renal vein or inferior vena cava and total resection of the renal tumor and IVC tumor thrombus is considered the optimal therapy when no distant metastases are present. The surgical approach to a specific patient with RCC and IVC tumor thrombus should be selected according to the level of the tumor thrombus and the characteristics of the primary renal tumor such as size, location, regional lymphadenopathy and aberrant vascular anatomy. The surgical management of RCC and IVC tumour thrombus requires the commitment of a multidisciplinary surgical team particularly for a higher (level III and IV) tumor thrombus. Preoperative imaging is very important for the surgical planning and management. An extended chevron incision with or without a sternotomy provides excellent surgical exposure. Level 0, level I and most level II tumors can be removed using straightforward occluding maneuvers, while maintaining control of the caval thrombus and the contralateral renal vein. Vascular bypass can frequently be required for the safe and complete resection of level III tumor thrombus. The resection of level IV tumors is generally performed with the use of cardiopulmonary bypass and circulatory arrest. In this review, these surgical aspects and treatment of IVC tumor extension in patients with RCC will be summarized, hoping to provide a clear and comprehensive guideline for both urology and cardiovascular specialists. Bu hastaların cerrahi tedavisi, özellikle yüksek seviyede (Seviye III ve IV) tümör trombüsü varlığında multidisipliner bir cerrahi takım çalışmasını gerektirir. Bu çalışmada atan kalpte koroner arter baypas greftleme (KABG) yapılan hastalarda anjiyografinin uzun dönem sonuçları değerlendirildi. Ça lış ma pla nı: Eylül 1993-Aralık 1995 tarihleri arasında aynı iki cerrah tarafından atan kalpte KABG yapılan ve takiben anjiyografisi çekilen toplam 162 hasta retrospektif olarak incelendi. Bul gu lar: Ameliyat ile anjiyografi arasında geçen süre ortalama 9.8±1.2 yıldı (dağılım 7.1-12.6 yıl). Genel olarak greft açıklık oranı sol internal mammaryan arterde %95.57 iken, safen ven greftinde %46.03 olarak bulundu. Takip sürecinde, 27 hastaya (%16.67) perkütan koroner girişim uygulandı. Bunların sekizinde SVG'de ciddi darlık nedeniyle, diğerlerinde nativ koroner arterde yeni ciddi lezyon oluşumu nedeniyle işlem uygulandı. Beş hastaya da tekrar KABG uygulandı. Ameliyat öncesi, sırası ve sonrası değişkenler ile greft açıklığı arasında anlamlı bir ilişki saptanmadı. So nuç: Konvansiyonel baypasa kıyasla, atan kalpte KABG tekniği kan ve kan ürünlerinin transfüzyonunu azaltma, yoğun bakım ünitesi ve hastanede kalış süresini kısaltma, maliyeti düşürme ve morbidite ve mortaliteyi azaltma gibi iyi bilinen avantajlara sahiptir. Çok uzun dönem anjiyografik takip çalışmamızda atan kalpte KABG'nin konvansiyonel baypas işlemi kadar güvenilir olduğu ve özellikle sol ön inen koroner arterde mükemmel sonuç ve açıklık sağladığı görüldü. Echinococcus granulosus infection may manifest with cardiac involvement causing various symptoms. Here, we discuss a case of a young patient who presented with sustained ventricular tachycardia (VT) causing hemodynamic compromise. Diagnosis of cardiac hydatid cyst was established with echocardiography, computed tomography imaging, and serological examination. After surgical removal of the cyst VT did not recur.
