IntroduçãoA amiloidose abrange um grupo de desordens de múltiplas etiologias devido a depósitos extracelulares de proteínas betafibrilares insolúveis, os depósitos amiloides que causam dano estrutural ao tecido.1 O termo "amiloyd" foi adotado em 1954 por Virchow com base na coloração por iodo e ácido sulfúrico do tecido. Todas as fibrilas amiloides compartilham uma mesma estrutura secundária, a conformação em folha betapregueada, e um componente não fibrilar idêntico, a pentraxina amiloide sérica P.A doença pode ser subdividida em: forma localizada (quando o material é depositado em um único órgão) e forma sistêmica (quando afeta mais de um órgão). O acometimento cardíaco isolado, em que o miocárdio se torna firme e menos complacente, é polimórfico e pode estar presente em qualquer tipo de amiloidose sistêmica, com ou sem manifestações clínicas associadas, sendo mais comum em idosos. 1 Na prática clínica, é classificada como amiloidose primária, secundária, hereditária, e é relacionada com a idade. A amiloidose primária aparece sem antecedentes ou doença coexistente que envolve órgãos mesenquimais, como os do sistema cardiovascular, trato gastrointestinal, e tecido muscular, sendo comum o envolvimento cardíaco. Já a amiloidose secundária está associada com doenças crônicas e tem uma tendência a depositar-se em órgãos parenquimatosos, como o fígado, o baço e os rins, sendo raro o envolvimento cardíaco. Por fim, a amiloidose hereditária é geralmente herdada de forma autossômica dominante. 1,2As formas de apresentação clínica da amiloidose cardíaca são variadas: insuficiência cardíaca, arritmias, embolia pulmonar e até morte súbita por fibrilação ventricular. As alterações anatomopatológicas observadas incluem o aumento na espessura parietal ventricular associado à textura miocárdica anormal, podendo haver envolvimento das valvas cardíacas pelo amiloide, sendo a valva mitral a mais acometida (90%), seguida da tricúspide (70%).3 Os achados na amiloidose cardíaca variam com a sua gravidade e duração.O eletrocardiograma (ECG) revela baixa voltagem em 50% dos casos, podendo apresentar também graus variados de bloqueios atrioventriculares e intraventriculares. O ecocardiograma (ECO) é um exame essencial para suspeita diagnóstica, pois é capaz de evidenciar o acometimento infiltrativo e restritivo, que são fatores sugestivos de amiloidose cardíaca. Ao ECO, observa-se textura miocárdica anormal difusamente brilhante com aspecto finamente "mosqueado". 4Nas fases iniciais, a textura anormal é sutil e padrões de fluxo de entrada no ecodopplercardiograma podem sugerir relaxamento retardado em vez de um padrão restritivo.5 Além disso, pode-se verificar a presença de derrame pericárdico e aumento atrial. 6 Pacientes com amiloidose e insuficiência cardíaca congestiva têm pior prognóstico.7 Na literatura, diversos autores descrevem marcadores prognósticos, que podem orientar o clínico.
