InleidingJanet (36) wordt opgenomen op een open afdeling van een verslavingskliniek met een rechterlijke machtiging (RM) op eigen verzoek. Janet is verslaafd aan cocaïne en heroïne en heeft daarnaast een borderlinestoornis. Ze is dakloos, prostitueert zichzelf en steelt om aan geld voor drugs te komen. Ze heeft een schuld van twintigduizend euro bij verschillende instanties. Ze is voorheen vaker vrijwillig opgenomen, maar beëindigde haar opnames steeds vroegtijdig en tegen advies. Janet lijkt de eerste dagen van de opname erg gemotiveerd en heeft grootse plannen. Maar na twee weken komt er verandering in de situatie. Ze laat zich steeds minder zien tijdens de programmaonderdelen en na drie weken loopt ze weg uit de kliniek. Vervolgens is ze twee dagen spoorloos, tot ze zich meldt bij de politie. Janet wordt teruggebracht omdat ze bij de politie gesignaleerd staat vanwege de RM. Ze blijkt twee dagen bij een dealer te hebben doorgebracht. De weken daarna loopt Janet herhaaldelijk weg uit de kliniek en komt dan uit eigen beweging terug -als haar geld op is -of wordt teruggebracht door de politie. Janet meldt zich namelijk bij het bureau met het verhaal: 'Ik heb een rechterlijke machtiging en ben opgenomen in de kliniek.' De politie brengt haar dan terug en ze wordt weer opgenomen. Nadat dit twee maanden zo verloopt, wordt Janet overgeplaatst naar de gesloten afdeling. Daar gaat het van kwaad tot erger. Janet weigert alle behandeling, loopt enkele keren weg, laat middelen naar binnen smokkelen en gebruikt op de afdeling. Patients with mental illnesses have been found to shorter life expectancy due to an increased risk of heart disease. Some medication used to treat mental illnesses have been linked to weight gain and other physical change that make patients susceptible to heart disease. In order to reduce this risk it is important that health professionals regularly measure and monitor signs of these physical changes. This research has found that measuring both waist circumference and blood pressure of patients is a safe and reliable way to way to monitor patients. To identify if combined blood pressure and waist circumference measurements are reliable predictor of metabolic syndrome, a descriptive correlational design was used to examine the sensitivity and specificity of screening techniques used to detect metabolic syndrome. Data were collected regarding waist circumference, body mass index, blood pressure, fasting blood glucose, triglycerides and high-density lipoproteins. Blood pressure and waist circumference measurements demonstrated high significance, sensitivity and specificity as screening instruments for metabolic syndrome. Combined waist circumference and blood pressure measurements may be clinically useful for a quick and reliable detection of metabolic syndrome in patients with addiction and comorbid mental health problems.

Recent use of alcohol, tobacco and hard drugs showed no statistical difference either.ConclusionPethidine for labour pain medication appears not to be associated with substance misuse or smoking in later life.
