ÖZETHerpes simpleks virüsü (HSV) sporadik viral ensefalitlerin en yaygın nedenidir. Ateş, bilinç bozukluğu ve fokal nörolojik bulgular HSV ensefalitinin tipik klinik özellikleridir. Tedavi edilmeyen olgularda mortalite oranı yüksektir. Uygun ve zamanında tedavi edilen olgularda bile kalıcı nörolojik sekeller görülebilmektedir. Kesin tanıda altın standart polimeraz zincir reaksiyonu (PCR) ile beyin omurilik sıvısı (BOS)'nda HSV-DNA'nın gösterilmesi olup, bu yöntemin duyarlılık ve özgüllüğü oldukça yüksektir. HSV ensefaliti tanısında beyin manyetik rezonans görüntüleme en iyi görüntüleme yöntemidir. Burada, beş gündür ateş ve şuur bozukluğu yakınmasıyla başvurup kliniğimizde HSV ensefaliti tanısıyla izlenen bir olgu sunulmuştur. BOS'da HSV-PCR pozitif ve serebral manyetik rezonans görüntülemede temporal alanda kanamalar saptanmıştır. Asiklovir tedavisine rağmen hastada kognitif bozukluklar ve polifaji gibi sekel bulgular görülmüştür. Sonuç olarak; HSV ensefalitinde erken tedavi ile mortalite ve morbidite azalabilir. Viral ensefalit düşünülen olgularda aksi ispatlanıncaya kadar HSV ensefaliti düşünülerek asiklovir başlanmalıdır.
ABST RACTHerpes simplex virus (HSV) is the most common viral cause of sporadic encephalitis. Fever, altered consciousness and focal neurologic signs are typical clinical features of HSV encephalitis. Untreated cases have high mortality. Even with appropriate and timely treatment, permanent neurologic sequelae can still be seen.
