Objetivo: las fracturas-luxaciones del carpo suponen un reto diagnóstico y de tratamiento. Un diagnóstico temprano y una reducción anatómica consiguen una mejoría de los resultados clínico-funcionales. El objetivo de este trabajo es presentar el caso de una fractura luxación de carpo inveterada y realizar un repaso del manejo de este tipo de lesiones. Material y métodos: se presenta el caso de un varón de 47 años que, tras una torsión, presenta una luxación transestiloperilunar que pasó inadvertida en el diagnóstico inicial de fractura de estiloides radial. A las 3 semanas se solicitó una tomografía computarizada debido a la persistencia de la tumefacción y la impotencia funcional. El paciente es diagnosticado de una luxación tranestiloperilunar. Resultados: se procede a reducción abierta y osteosíntesis mediante tornillos Acutrack®. Se mantiene férula durante 8 semanas. Retirada del material de osteosíntesis a las 8 semanas. El objetivo es conseguir una pseudoartrosisis (reduction and association of the scaphoid and lunate-RASL-), ya que consigue mejor control del dolor, amplitud de movimiento y fuerza de agarre que con una artrodesis exitosa. Conclusión: es importante reconocer y tratar estas lesiones de forma temprana para obtener los mejores resultados. Si no se diagnostican, la inestabilidad carpiana tiene como consecuencia una limitación progresiva del movimiento, artrosis degenerativa y dolor crónico.
