O novo coronavírus foi primeiramente reconhecido em Wuhan, China, em dezembro de 2019, causando a doença do coronavírus 19 (COVID-19). No dia 11 de março de 2020, a Organização Mundial de Saúde declarou se tratar de uma pandemia. O curso clínico da COVID-19 pode ser classificado em três estágios: “infecção precoce”, “fase pulmonar” e “fase de hiperinflamação”, cada uma caracterizada por alterações bioquímicas específicas. O objetivo do trabalho foi de descrever as diferenças e o potencial prognóstico de exames de sangue de rotina em pacientes hospitalizados em enfermaria e unidade de terapia intensiva (UTI) com diagnóstico de COVID-19. Trata-se de um estudo longitudinal e retrospectivo, cuja coleta de dados se deu por meio das informações contidas em prontuário eletrônico. Durante o período de estudo, foram encontrados 62 pacientes com infecção confirmada por SARS-CoV-2; destes 67,78% internaram em enfermaria. Entre os pacientes tratados em UTI, idade, leucócitos, bastões, neutrófilos, creatinina, bilirrubina total, LDH e D-dímero foram maiores do que entre os que permaneceram na enfermaria. Por outro lado, hemoglobina e hematócrito nos pacientes internados em UTI foram menores. Acreditamos que os níveis de leucócitos na admissão do paciente com COVID-19 podem indicar a gravidade da doença e que acompanhar os níveis de hemoglobina, LDH e D-dímero durante a internação do paciente pode ser importante para avaliar a progressão da doença.
A pandemia da doença respiratória causado por um Novo Coronavírus (denominado "SARS-CoV-2”) ganhou atenção global e foi reconhecida como uma séria ameaça à saúde pública mundial. O objetivo do estudo é de caracterizar o perfil demográfico e clínico de pacientes com diagnóstico de COVID-19, em um hospital público de referência no Paraná. Estudo transversal, com levantamento de dados em prontuários de pacientes internados suspeitos ou confirmados em um hospital de referência para atendimento da COVID-19, no Paraná, no ano de 2020. As comparações da variável dependente COVID-19 com as características clínicas, laboratoriais e demográficas foram analisadas pelos testes U de Mann-Whitney, t de Student ou qui-quadrado, adotando-se um nível de significância de 5%. No ano de 2020, foram internados 255 pacientes entre suspeitos e confirmados com COVID-19. Do total, 185 (72,5%) tiveram diagnóstico de COVID-19 confirmado por RT-PCR, 58 (22,7%) negativos e 6 (2,4%) não foi encontrado o resultado. Identificamos associação com casos confirmados de COVID-19, valores elevados de proteína C reativa, ferritina, e transaminase pirúvica (TGP), bem como dos valores menores de ureia, creatinina, D-dímero, peptídeo natriurético (BNP). Além disso, a menor frequência cardíaca foi identificada como fator associado com os casos de COVID-19. É fundamental conhecer o histórico, a apresentação clínica e o perfil bioquímico da COVID-19, com o intuito de identificar precocemente casos graves, possibilitar a estratificação de risco e a aplicação de uma abordagem direcionada e específica, prevenindo desfechos desfavoráveis.
