An analysis of birthweights of 337 neonates in relation to history of maternal narcotic usage was undertaken Mean birthweight of infants born to mothers abusing heroin during the pregnancy was 2,490 gm, an effect primarily of intrauterine growth retardation. Low mean birthweight (2,615 gm) was also seen in infants born to mothers who had abused heroin only prior to this pregnancy, and mothers who had used both heroin and methadone during the pregnancy (2,535 gm). Infants born to mothers on methadone maintenance during the pregnancy had significantly higher mean birthweights (2,961 gm), but lower than the control group (3,176 gm). A highly significant relationship was observed between maternal methadone dosage in the first trimester and birthweight, i.e., the higher the dosage, the larger the infant. Heroin causes fetal growth retardation, an effect which may persist beyond the period of addiction. Methadone may promote fetal growth in a dose-related fashion after maternal use of heroin.
Our clinical data enabled us to demonstrate a correlation between impaired lipid metabolism and vasculogenic impotent men. Our aim was to evaluate the effect of an impaired lipid metabolism on the smooth muscle of the corpus cavernosum. A total of 16 rabbits were given a cholesterol-enriched diet for 3 months, and 8 of these received additional thromboxane A2 receptor antagonist; 10 other rabbits (control) were fed a normal diet. Subsequently, cavernous tissue biopsies were taken, and tissue lipid extractions and electron microscopic evaluation were made from 3 rabbits in each group. In the untreated high-cholesterol diet group, cholesterol levels reached approx. 2.1 micrograms/mg body weight compared with 1.07 micrograms/mg b.wt. in the thromboxane A2 receptor antagonist-treated group and elevated levels compared with control group. Similar results were found for the triglyceride and free fatty acid levels. Lecithin tissue levels in treated rabbits were distinctly elevated against those of other 2 groups. Ultramorphological examination of the control group disclosed normal smooth muscle cell (SMC) architecture with numerous sites of intercellular contacts. These findings contrasted with those of the high-cholesterol diet groups which showed significant SMC degeneration with loss of intercellular contacts. Our data imply that impaired lipid metabolism causes cavernous SMC degeneration which plays a major role in the pathogenesis of erectile dysfunction. The thromboxane A2 receptor antagonist seems to produce a protective metabolic effect on the erectile tissue which may have some consequences future treatment strategies.
Various aspects of the detoxification of rehabilitated methadone patients are considered in the light of experience at the Albert Einstein College of Medicine-Bronx Psychiatric Center MMTP and the reports of others in the field. Patients studied met certain eligibility requirements which were thought to enhance the probability of successful detoxification. A total of 228 (10.4%) of 2,814 patients were included in the project. Sixty-three patients have completed detoxification. A follow-up on these patients indicates that 22.2% claim abstinence from all drugs and are reporting to the clinic, 47.6% claim abstinence but have not reported to the clinic, 14.3% returned to methadone maintenance, and 15.9% are lost to contact. This and other studies emphasize the importance of adequate counseling during detoxification.
