Introdução: Amiloidose AL é uma doença associada à deposição tecidual de amiloides, derivados de cadeias leves de imunoglobulinas clonais, produzidas por plasmócitos na medula óssea. É uma doença rara e subdiagnosticada, sendo necessária a maior consciência clínica sobre a sintomatologia e os exames diagnósticos, para uma identificação precoce com melhor prognóstico. Objetivo: Determinar os principais sintomas clínicos e exames complementares para Amiloidose AL, para diagnóstico diferencial em relação a demais patologias. Materiais e Métodos: Leitura, análise e discussão de artigos científicos encontrados nas bases científicas: PubMed e Scielo, publicados no período de 2015 a 2022, usando como descritores: "amyloidosis", "delay", "diagnosis", "treatment". Resultados: Embora os sintomas sejam comuns a outras patologias, há 4 cenários clínicos suspeitos para amiloidose relacionados aos sistemas renal, cardíaco, hepático e neurológico. Além disso, a macroglossia e a púrpura periorbital são achados clínicos importantes para amiloidose AL. O teste padrão ouro para amiloidose é a aplicação do corante vermelho congo em uma amostra de tecido e, para a determinação da tipagem AL, os exames indicados são a eletroforese sérica e urinária com imunofixação. Conclusão: O diagnóstico tardio da doença está associado a um risco aumentado de morte. Tendo em vista os avanços no tratamento da amiloidose, é necessário que o tempo de diagnóstico acompanhe tal evolução, a fim de melhorar a sobrevida e aumentar a qualidade de vida do paciente.
