Diabetes Mellitus tipe-1 merupakan salah satu penyakit kronis yang sampai saat ini belum dapat disembuhkan. Walaupun demikian, berkat kemajuan teknologi kedokteran kualitas hidup pasien DM tipe-1 dapat terpelihara. Sebagian besar kasus DM pada anak termasuk di dalam DM tipe-1, namun sedikit sekali tulisan di Indonesia mengenai karakteristik klinis dan laboratorium DM tipe-1. Hal ini mengakibatkan banyak tenaga medis yang tidak mengetahuinya sehingga sering sekali salah dalam mendiagnosis pasien DM tipe-1. Pasien DM tipe-1 sebagian besar mempunyai riwayat perjalanan klinis, dengan poliuria, polidipsia, polifagia, dan berat badan yang cepat menurun. Untuk mengurangi keterlambatan diagnosis maka kewaspadaan terhadap DM tipe-1 merupakan kata kunci. Telah dilakukan penelitian deskriptif retrospektif pada semua pasien DM tipe-1 yang berobat ke Bagian IKA-RSCM antara tahun 1989-1999. Seluruh subyek penelitian diambil dari catatan medis dan didapat 41 kasus dengan DM tipe-1 dengan sebagian besar anak perempuan. Usia terbanyak saat datang pertama kali ke Bagian IKA-RSCM pada usia 5-10 tahun (56%). Dari 41 kasus DM tipe-1, hanya 31% yang mempunyai keturunan keluarga DM. Saat datang ke Bagian IKA-RSCM 66% dengan katoasidasis diabetik, 71% menderita gizi kurang dan seluruh pasien datang dengan keluhan poliuria, polidipsia, polifagia dan adanya penurunan berat badan yang progresif. Gangguan kesadaran juga menyertai pasien dengan KAD. Kadar gula darah pasien DM tipe-1 terbanyak antara 300-500 mg/dl (51%). Semua pasien pada saat awal diagnosis kadar HbA1c di atas normal dan C-peptida di bawah normal.Kata kunci: DM tipe-1, poliuria, polidipsia, polifagia, ketoasidosis diabetik. Diabetes mellitus (DM) adalah kelainan yang bersifat kronis ditandai dengan gangguan metabolisme karbohidrat, protein dan lemak yang disebabkan defisiensi insulin baik absolut dan atau relatif.1 Defisiensi insulin absolut biasanya didapatkan pada pasien diabetes mellitus tipe-1. Hal ini disebabkan adanya kerusakan sel b pankreas yang progresif sehingga insulin tidak dapat disintesis oleh kelenjar pankreas. Defisiensi insulin relatif ditemukan pada pasien DM tipe-2 oleh karena pemakaian insulin di dalam tubuh kurang efektif.2 Sebagian besar kasus DM pada anak termasuk dalam DM tipe-1, yang terjadi akibat suatu proses autoimun yang merusak sel b pankreas sehingga produksi insulin berkurang bahkan berhenti. Oleh karena itu pasien sangat tergantung pada insulin untuk kelangsungan hidupnya. Diabetes mellitus tipe-1 ini disebut juga DM tergantung insulin (DMTI).Klasifikasi DM berdasarkan etiologi mempunyai 4 bentuk klinis yaitu DM tipe-1, DM tipe 2, DM tipe lain dan gestational diabetes mellitus. DM tipe-1 merupakan tipe DM terbanyak pada anak dan didapatkan di berbagai negara termasuk Indonesia. BackgroundHypertension is an uncommon manifestation of congenital adrenal hyperplasia due to 21-hydroxylase enzyme deficiency (21-OHD CAH). Nevertheless, it is necessary to continuously be aware of hypertension incidence in 21-OHD CAH patients and its possible causes. Conflicting results regarding the role of PRA have been reported. This study aims to evaluate the association between plasma renin activity levels (PRA) with hypertension in 21-OHD CAH children. MethodsWe did an observational analytic study with a cross-sectional study design. The subjects were 21-OHD CAH children, aged >6 months to 18 years, selected by consecutive sampling and then compared between 21-OHD CAH with hypertension and non-hypertension groups. We gathered 40 subjects, 20 subjects in hypertension and 20 subjects in non-hypertension groups.ResultsThere were 16 from 27 (59.3%) and 4 from 13 subjects (30.8%) with hypertension in salt-wasting and simple virilizing type, respectively. There was a significant mean difference in PRA levels between hypertension and non-hypertension groups in salt wasting patients (p=0,016). The risk of hypertension in salt wasting patients with low PRA levels was 1,09 times after controlling for sex, 17-OHP levels, and the last fludrocortisone dose. The last dose of hydrocortisone had a significant relationship with the incidence of hypertension in salt wasting patients. ConclusionsThis study concludes that hypertension in 21-OHD CAH patients during the study period was 32%. The risk of hypertension in salt wasting patients with low PRA levels was 1.09 times after being controlled for sex, 17 OHP level, and the last dose of fludrocortisone.
Latar belakang. Insufisiensi adrenal sering dijumpai pada pasien syok septik dan berhubungan dengankejadian syok refrakter katekolamin dan peningkatan mortalitas.Tujuan. Mengetahui profil fungsi adrenal pada anak yang menderita sepsis di Unit Perawatan IntensifPediatrik RSCM.Metode. Penelitian deskriptif dengan desain potong lintang. Semua subjek menjalani uji stimulasikortikotropin dosis standar. Insufisiensi adrenal (IA) ditegakkan jika respons peningkatan kortisol terhadapuji ( maks) 9 μg/dl dan dikelompokkan menjadi absolut (IAA) jika kortisol pra-uji t0 <20 μg/dl danrelatif (IAR) jika t0 20 μg/dl.Hasil. Tiga puluh anak sepsis (18 laki-laki) diikutsertakan dalam penelitian. usia median adalah 29 (1-153)bulan dan median skor PELOD 12,5 (0-33). Insufisiensi adrenal dijumpai pada 8 (26,7%) pasien (2 IAA and6 IAR), lebih sering pada perempuan (p=0,003), serta cenderung lebih sering pada syok septik (p=0,682).Median t0 32,75 (4,23-378) μg/dl, t30-60 48,20 (16,70-387) μg/dl, dan maks 15,40 (0,90-60,80) μg/dl. Kadar t0 dan t30-60 lebih tinggi pada kelompok syok septik (p=0,03 dan p=0,01) namun tidak berbedaantara subjek dengan dan tanpa IA. Terdapat kecenderungan korelasi positif antara skor PELOD dengant0 dan t30-60 (r=0,7; dan r=0,6; p0,001). Hipotensi sistolik dijumpai pada seluruh subjek dengan IA dansyok refrakter katekolamin cenderung lebih sering pada kelompok IA (p=0,67). Mortalitas 36,7%, tidakberbeda antara subjek dengan dan tanpa IA.Kesimpulan. Insufisiensi adrenal sering dijumpai pada yang menderita anak sepsis dan harus dipertimbangkanpada kondisi syok septik. Penelitian lanjutan diperlukan untuk menentukan efektivitas terapi steroid.
