Latar belakang: suatu kasus impaksi gigi dapat menyebabkan maloklusi, dan kelainan oklusi akan semakin bertambah dengan bertambahnya usia. Impaksi gigi kaninus merupakan gigi kedua setelah gigi molar ketiga yang berfrekuensi tinggi untuk mengalami impaksi, persentasenya sekitar 12%-15% dari populasi. Gigi supernumerary adalah gigi tambahan/berlebih, sehingga jumlah gigi yang terbentuk dalam rahang lebih banyak dari jumlah normal. Terjadinya impaksi gigi kaninus dan supernumerary secara bersamaan jarang terjadi. Tujuan: Menambah wawasan du bidang bedah mulut minor, terutama dalam menangani suatu kasus impaksi gigi kaninus dan supernumerary untuk perawatan orthodonti. Kasus dan Penanganan: Dilaporkan seorang apsien, wanita, berusia 38 tahun yang baru menyadari kelainan maloklusinya dengan keluhan gigi depannya bertambah maju akibat adanya impaksi gigi kaninus dan impaksi gigi supernumerary. Pasien dikonsulkan dari Bagian Orthodonti ke Bagian Bedah Mulut untuk penanganan impaksi gigi kaninus dan supernumerary dengan kemungkinan untuk mempertahankan gigi kaninus memulai pembedahan. Dengan berbagai pertimbangan, penderita pada akhirnya menjalani operasi pengambilan gigi kaninus dan gigi supernumerary di Bagian Bedah Mulut RSGM Prof. Soedomo. Langkah-langkah diagnosis, operasi dan berbagai kemungkinan komplikasi juga turut disertakan di dalam pembahasan. Kesimpulan: Pengambilan gigi kaninus dan gigi supernumerary yang terpendam merupakan pilihan perawatan jika tidak memungkinkan untuk dilakukan exposure pada impaksi gigi kaninus pada maksila. Background: Impacted canines is the second most impacted tooth after third molar impaction, approximately 12%-15% of the population present with impacted canines. A supernumerary tooth is one that is additional to the normal series and can be found in almost any region of the dental arch. The incidence of an impacted canines as a sequent with a supernumerary tooth is very rare. Purpose: The aim of this case report is to add more information about a minor surgery due to canine and supernumerary tooth impaction for orthodontic treatment. Case and Management: We reported a case of a woman, 38 years old who have noticed a malocclution through the forwardness movement of her anterior teeth, due to the present of impacted canine and supernumerary. The patient consulted from orthodontic department to oral and maxillofacial department for further assessment, treatment and also the probability for surgical exposure of impacted canine. We have decided to do odontectomy for the impacted canine and supernumerary tooth as well at Oral and Maxillofacial department, Prof. Soedomo Hospital. The diagnosis process, exposure of impacted canine considerations are also discussed. Conclusion: the odontectomy for impacted canine and supernumerary teeth had performed as last options if there is impossible to do an exposure of an impacted canine.
ABSTRAKKasus bibir dan lelangit sumbing merupakan salah satu kelainan deformitas yang sering terjadi. Keadaan klinis bervariasi mulai dari kasus sumbing tidak komplit sampai dengan komplit. Keadaan klinis dengan lebar celah yang bervariasi membutuhkan teknik pembedahan yang tepat. Pasien-pasien dengan bibir dan atau lelangit sumbing mungkin sebelumnya telah menjalani beberapa intervensi pembedahan, sehingga sering memerlukan koreksi lebih lanjut untuk memperbaiki hasil operasi sebelumnya. Pasien anak laki-laki usia 4 tahun 7 bulan mengeluhkan masih terdapatnya celah di lelangit. Pasien didiagnosis dengan labiognatopalatoschisis bilateral. Sebelumnya pasein sudah menjalani 4 kali operasi penutupan celah bibir dan lelangit. Pasien menjalani 2 kali operasi bibir sumbing dengan metode lip adhesion dan metode Barsky, dan 2 kali operasi lelangit sumbing dengan metode pushback dilanjutkan dengan koreksi dengan z plasty, tetapi hasil akhir masih terdapat celah di palatum mole. Selanjutnya dilakukan operasi repair palatoplasi dengan metode Furlow double opposing z plasty dengan kombinasi insisi lateral, dan didapatkan hasil menutupnya celah di palatum mole sampai dengan uvula. Telah dilakukan operasi repair palatoplasi dengan metode Furlow double opposing z plasty. Teknik ini dilakukan untuk menghindari insisi yang terlalu luas dikarenakan terdapatnya jaringan fibrous yang tebal pada mukosa palatum pasca operasi sebelumnya. Tujuan studi kasus adalah untuk mengetahui kemampuan teknik Furlow Double Opposing Z Plasty sebagai prosedur repair palatoplasty. Maj Ked Gi Ind. There are variety of clinical appearance ranging from incomplete to complete cases. Clinical appearance with different width requires proper surgical technique. Patients with cleft lip and palate had undergone surgical intervention, so that they needed surgical correction to repair the result or failure of the previous surgery. A Four year old boy complain there was cleft on the soft palate. The patient was diagnosed with labiognatopalatoscisis. The patient had undergone two stages of cleft lip surgery and twice of cleft palate surgery with pushback method and repair with z plasty, however the result was unsatisfactory. Further, the patient underwent repair palatoplasty surgery with Furlow double opposing z plasty method combined with lateral relaxing insicion. The result in the post surgery was the closure of cleft soft palate up to uvula. Repair palataplasty surgery has been done with Furlow double opposing z plasty method. This technique could avoid extended incision due to thick fibrous tissue on the palatum mucosa as the result of serial previous surgery. Latar Belakang: Epulis merupakan hyperplasia gingival yang menyerupai tumor (tumour like) yang berasal dari jaringan ikat selaput periodontal/ parenkim yang disebabkan karena iritasi kronis. Epulis fibromatosa memiliki penampakan klinis bertangkai (peduncullated), dapat pula tidak, warna agak pucat konsistensi kenyal, batas tegas, padat, kokoh, tidak mudah berdarah dan tidak menimbulkan rasa sakit. Tujuan: mengevaluasi hasil perawatan ekstirpasi epulis fibromatosa ukuran besar dengan anestesi lokal. Kasus: ekstirpasi epulis fibromatosa pada kasus ini dilakukan pada wanita usia 50 tahun yang datang dengan kesulitan makan karena benjolan 5x4x3 cm di dalam mulut pada pipi sebelah kiri. Penatalaksanaan: ekstirpasi epulis fibromatosa dengan anestesi lokal pada kasus ini juga dilakukan penghilangan faktor predisposisinya berupa pembersihan kalkulus sebelum operasi dan penghalusan tulang yang tajam yang merupakan iritasi kronis lokal. Kesimpulan: perawatan ekstirpasi epulis berukuran besar dengan anestesi lokal pada kasus ini bisa dipertimbangkan, karena masi ada akses untuk dilakukan deponir larutan anestesi. Background: Epulis is a gingival hyperplasia that characteristic likes tumor, devided from the periodontal connective tissue/ parenchyma due to chronic irritation. Epulis fibromatosa has peduncullated clinical appearance, or may not, the color is rather pale than surrounding tissue, elastic consistency, well devined, solid sturdy, no bleeds easily and does not cause pain. Objective to evaluate the extirpation result of large epulis fibromatosa with lokal anesthesia. Case the extirpation on this case treatment is performed on woman aged 50 years old who came found it difficult to eat because of 5x4x3 cm lump in the mouth on the left cheek. Treatment the extirpation management of epulis fibromatosa with lokal anesthesia in this cases also done by predisposition factors removal, such as calculus removing before surgery and smoothing the sharp bone that make a local chronic irritation. Conclusion a large epulis extirpation treatment with local anesthesia in this case could be considered, because there is access to inject anesthetic solution.
