Die postoperative (p. o.) Bronehialfistel belastet noch immer die moderne Lungen chirurgie, und es darf keine Mühe gescheut werden, um diese schwerste, häufig töd liche Komplikation der Lungenresektionen endgültig zu beherrschen. Die Wichtig keit des Themas steht allerdings in keinem Verhältnis zu der geringen Beachtung, die es bisher in der medizinischen Literatur gefunden hat. Seit dem großen Bericht der Französischen Gesellschaft für Thoraxchirurgie im Jahre 1951 (16, 28), der auch über die ältere Bibliographie unterrichtet, haben wir nur 4 Arbeiten 1) auffinden können, die sich eingehend mit dem Thema befassen (6, 11, 17, 24), gewöhnlich wird es nur am Rande gestreift. Es sei uns daher erlaubt, das Thema neu aufzugreifen, in der Hoffnung. damit einen Beitrag zur Verhütung dieser katastrophalen Komplikation zu leisten. Definition Wir verstehen unter dem Begriff ,,p.o. Bronehialfistel" eine pathologische Kom munikation zwischen Pleura-bzw. Thoraxraum und Bronchialsystem, die sich nach einer Lungenresektion ausgebildet hat, zum Dauerzustand geworden ist und sich selbst überlassen keine Tendenz zur Heilung zeigt. Etwas anderes sind unseres Erachtens ,,Pulmonalfistein'', bei denen das Lungenparenchym Sitz einer Fistel ist und kein makroskopischer Bronchusdefekt besteht. Sie dauern gewöhnlich kurze Zeit und heilen meist spontan ab. Obgleich sie mit den eigentlichen Bronchialfisteln nichts zu tun haben, sind sie doch aus differentialdiagnostischen Gründen von Bedeutung. Unsere 5 einschlägigen Fälle werden am Schluß kurz besprochen. Chirurgisch-thera peutisches Interesse verlangte nur ein Fall. Einteilung Im allgemeinen teilt man die Bronchialfisteln nach zeitlichem oder anatomischem Gesichtspunkt ein. Der letztere, nach dem zwischen Segment-, Lappen-und Hauptbronchusfisteln unterschieden wird, ist vor allem klinisch-therapeutisch wichtig, da die Symptome um so stürmischer sind, je größer der befallene Bronchus ist, und die Behandlung sich verschieden gestalten kann, je nachdem, ob noch Lungengewebe *) Die Arbeiten von Angulo (Beit. Klin. Chir. 189, 76-89 [1954]) und Peitsch (Zeitsch.Tuberk. 107, 53-58 [1955]) gelangten erst nach Abschluß des Manuskriptes zur Kenntnis der Autoren und konnten daher nicht berücksichtigt werden.
