Marqueurs cliniques des troubles immunitaires dans la pathogenèse de la coli entérite Introduction. La coli entérite est l'une des causes les plus courantes de diarrhée dans les pays développés et est généralement causée par des souches pathogènes d'Escherichia coli. Objectif: étudier le rôle de la réponse réactive des granulocytes neutrophiles polymorphonucléaires (GN) du sang périphérique dans les mécanismes de réponse inflammatoire systémique de l'entérocolite aiguë d'Escherichia (AEC) en fonction du polymorphisme des gènes de la protéine de choc thermique (HSP) de la famille 70-2 (HSP70-2, 1267A®G) et l'interleukine 10 (IL-10, C-592A). Méthodes. Le polymorphisme des gènes a été analysé par la méthode basée sur la PCR chez 95 patients avec AEC et 30 individus en bonne santé. Material and methods. The genes polymorphism was analysed by PCR based method in 95 patients with AEC and 30 healthy individuals. Ешерихіозні ентероколіти відносять до гострих кишкових інфекцій. Патогенні кишкові палички володіють прямою та опосередкованою пошкоджуючою дією через розвиток екзо-та ендогенної інтоксикації, запускається каскад захисних імунологічних механізмів. Мета роботи. Дослідити імуногематологічні показники ендогенної інтоксикації та клітинної реактивності у хворих на діарейний ешерихіоз. Матеріал і методи. Дев'яносто п'ять хворих на гострий ешерихіозний ентероколіт (ГЕК) пройшли скринінг і взяли участь у дослідженні. Серед них 33 (34,74%) чоловіки і 62 (65,26%) жінки, середній вік склав (38,66±3,11) років. Розрахунок гематологічних індексів і коефіцієнтів ендогенної інтоксикації та клітинної реактивності провели на підставі розширеного загальноклінічного аналізу крові. До групи контролю увійшли 30 практично здорових осіб. Результати. У хворих на ГЕК встановили зниження активності факторів і механізмів неспецифічного протиінфекційного захисту та загальної резистентності організму у відповідь на супресивну дію інтоксикації бактеріального походження та розвиток запального процесу: за зменшенням лімфоцитарно-гранулоцитарного індексу на 27,81%, зростанням індексу зсуву лейкоцитів на 47,44%, індексу зсуву нейтрофілів у 3 рази, зростанням лейкоцитарного індексу інтоксикації на 50,41%. Ядерний індекс ступеня ендотоксикозу та гематологічний показник інтоксикації стали вищі за такі у групі контролю в 6,3 і 3,82 раза відповідно (р<0,01). Зменшився загальний рівень неспецифічної резистентності на тлі вираженої активації мікрофагальної системи протиінфекційного захисту (зріс на 88,33% індекс співвідношення нейтрофілів і моноцитів) порівняно з активністю макрофагальної захисної системи. Висновки. Виснаження факторів і механізмів неспецифічної реактивності, активності нейтрофільних гранулоцитів у хворих на гострий ешерихіозний ентероколіт супроводжується формуванням імунної відповіді-зниженням імунологічної резистентності на тлі зростання імунологічної реактивності у 2,6 раза.
